The main objective of this study was to investigate the long-term effect of renal artery sympathetic ablation and its complications in patients with treatment-resistant hypertension. In this prospective study which done between March 2012 and November 2013, 30 patients with resistant arterial hypertension despite treatment with ≥3 antihypertensive drugs-were randomly enrolled in this self-control clinical study in Isfahan, Iran. This revision is the result of an active search for scientific articles with the term "renal denervation" in the Medline and PubMed databases. The techniques and devices used in the procedure are presented, as well as clinical outcomes at six, 12 and 24 months after the intervention with the Symplicity catheter. Significant decreases and progressively higher reductions of systolic and diastolic blood pressure were observed after RSD. The complication rate was minimal. New devices for RSD and its ongoing clinical studies are cited. In conclusion, the RSD presents itself as an effective and safe approach to resistant hypertension. Results from ongoing studies and longer follow-up of these patients are expected to confirm the initial results and put into perspective the expansion of the procedure use in hypertension approach. A reciprocal association between the brain and the kidney has been described, in that sympathetic overactivity affects renal function while renal injury stimulates central sympathetic drive. Renal nerve ablation has been recently introduced as a potential alternative for the management of resistant hypertension, mainly due to current limitations in pharmacologic antihypertensive therapy. Data accumulated thus far point towards an efficacious and safe interventional method for the management of treatment resistance, with additional benefits on glucose metabolism and cardiac structure and function. Furthermore, beneficial effects have been observed in patients with chronic kidney disease, obstructive sleep apnea, polycystic ovary syndrome, and sympathetically driven tachyarrhythmias. Activation of the sympathetic nervous system is an important etiological factor in the pathogenesis of hypertension and renal sympathetic nervous system is one of its pivotal arm. Thus, renal sympathetic system was the target of therapy for hypertension in the initial period, in the form of sympathectomy. The major adverse events associated with this procedure and the emergence of potent blood pressure (BP) lowering medications led to disrepute of this procedure. Of late, novel techniques targeting the sympathetic nervous system have emerged. Among them and the most promising one is catheter-based radiofrequency ablation of renal nerves-Renal sympathetic denervation (RSD). Clinical trials of renal denervation therapy have shown consistent and durable reduction of systolic as well as diastolic BP persisting up to 3 years. Beyond BP control, renal denervation also exerts favorable effects on glucose metabolism, heart failure, and sleep apnea. As many as five different catheter-based renal denervation systems are now approved for treating resistant hypertension, the maximum clinical experience being with Medtronic's symplicity system. The European Society of Cardiology guidelines for hypertension 2013 now recommend catheter-based renal denervation for management of drug-resistant hypertension. This study was designed to assess the safety and effectiveness of the OneShotª Renal Denervation System. RAPID is a prospective, multicentre, single-arm study which enrolled 50 patients at 11 clinical sites in Europe and New Zealand. Eligible patients had an office systolic blood pressure (SBP) ≥160 mmHg and were on a stable regimen of ≥3 antihypertensive medications including a diuretic. The primary safety endpoints were acute procedural safety at discharge and chronic procedural safety at six months. The primary effectiveness endpoint was the rate of office SBP reduction ≥10 mmHg at six months compared to baseline. While not a predefined endpoint, change in 24-hour ambulatory BP was evaluated. Increased sympathetic outflow to the heart, resulting in increased cardiac output and neurally mediated vasoconstriction of peripheral blood vessels, is an obvious example of a neural pathophysiologic pathway leading to elevated blood pressure. The consequences of increased sympathetic outflow to the kidneys, perhaps most important in this context, are sodium and water retention, increased renin release, and alterations of renal blood flow-effects that contribute substantially to both acute and long-term blood pressure elevations. Accordingly, renal sympathetic nerve ablation appears to be a logical therapeutic approach for the treatment of hypertension. Additionally, it aims to examine current evidence supporting its clinical use, discuss indications and contraindications, and identify areas where further research is needed to improve understanding and clinical practice. Methodology: A bibliographic review was carried out through searches in the PubMed Central (PMC) and SciELO (Scientific Electronic Library Online) databases. 196 articles were found and, after applying the inclusion criteria, 24 studies were selected to compose the collection. Results: Several types of studies reviewed indicate that Renal Sympathetic Denervation (RSD) is effective and safe in the management of resistant hypertension, both as a stand-alone treatment and as adjunctive therapy. The effects of this treatment demonstrated a reduction in blood pressure for at least 6 months after the procedure, with no complications and no kidney damage for at least 7 years after the intervention. The patient described here had a seven-year history of hypertension and presented with poorly controlled blood pressure despite antihypertensive therapy with four different drugs. A 44-yr-old man underwent percutaneous renal denervation under local anesthesia using an ablation catheter. After six months of follow-up his blood pressure had dropped 49/37 mmHg with a decrease in 24-hr ambulatory BP of 20/18 mmHg. Renal Doppler ultrasound showed no significant stenosis in either renal artery. Afferent signaling from the kidneys has been identified as an important contributor to elevated central sympathetic drive and increased sympathetic outflow to the kidney and other organs is crucially involved in cardiovascular control. While the resultant effects on renal hemodynamic parameters, sodium and water retention, and renin release are particularly relevant for both acute and long term regulation of blood pressure, increased sympathetic outflow to other vascular beds may facilitate further adverse consequences of sustained sympathetic activation such as insulin resistance, which is commonly associated with hypertension. Recent clinical studies using catheter-based radiofrequency ablation technology to achieve functional renal denervation in patients with resistant hypertension have identified the renal nerves as therapeutic target and have helped to further expose the sympathetic link between hypertension and insulin resistance.
