Positive blood cultures may help provide clinical diagnosis, as well as a specific etiological diagnosis. Neonatal septicemia constitutes an important cause of morbidity and mortality amongst neonates in India. However, with presently available antimicrobial agents, neonatal septicemia may be treated successfully. Early diagnosis and proper management of neonatal septicemia can bring down the morbidity and mortality substantially. Hence, the present study was undertaken to study the bacteriological profile of neonatal septicemia cases and their antimicrobial sensitivity pattern for planning strategy in the management of these cases. Materials and Methods: This retrospective study was carried out in the CNBC and MYH, department of microbiology and paediatrics, M.G.M. medical college and M.Y. hospital, Indore, from January to December in 1000 blood cultures. Identification of organism was done as per standard methods like gram’s staining, cultural characteristics, motility and biochemical reactions. Results: In our study done on 1000 neonatal blood cultures, we found 42.2 % cultures were positive, in which 51.3 % were males and 48.7 % were female neonates. We found 52.8 % were Early onset septicaemia (EOS) (\u0026amp;lt;2 days) and 47.2 % were late onset septicemia (LOS) (\u0026amp;gt;2 days). Among 1000 neonates 543 were institutional delivered (inborn) and 457 were out born referred from other hospitals; 64.7 % were term, 35.3 % were preterm, 80.1 % were normal deliveries and 19.9 % were LSCS. Ltd.</a></div><p class="ds-related-work--metadata ds2-5-body-xs">Innovative publication, 2016</p><p class="ds-related-work--abstract ds2-5-body-sm">Background: Neonatal sepsis is a clinical syndrome which is characterised by signs and symptoms of infection which may or may not be accompanied by bacteraemia within the first month of life. The bacteriological profile of neonatal sepsis is under constant change with the advances in the diagnosis and treatment. Blood culture is still the mainstay of investigation of potential sepsis in spite of the recent advances in molecular diagnosis of fungal and bacterial sepsis. Most common bacterial organisms responsible for neonatal septicaemia in developing countries like Bangladesh, India, Nigeria, Pakistan, are Klebsiella, Escherichia coli, Enterobacter etc. The present study was undertaken to describe the spectrum of the isolates in cases of neonatal septicaemia, and their antimicrobial susceptibility pattern. The study was carried out over a one year period. Method: A total of 357 suspected patients of neonatal septicaemia were included in the study. Blood samples were collected with aseptic precautions preferably prior to the initiation of empiric antibiotic therapy. The bottles were incubated aerobically at 37 0 C for 7 days and subcultured on blood agar and Mac Conkey agar. If growth appeared the isolates were processed as per standard microbiological techniques and the isolates were identified. Antibiotic sensitivity testing was performed on Mueller‑Hinton agar plates by modified Kirby‑Bauer disk diffusion method as per Clinical Laboratory Standard Institute guidelines. Results: Of the 357 samples studied, growth was observed in 154 samples. Gram positive organisms constituted 37.66% and gram negative bacilli constituted 62.34% of the isolates. The predominant pathogens isolated were Klebsiella species, Coagulase Negative Staphylococci, Staphylococcus aureus followed by Non fermenting gram negative bacilli. 71.43% of the Staphylococcus aureus isolates were found to be methicillin resistant.96.05% of the Enterobacteriaceae were found to be Extended Spectrum Beta Lactamase (ESBL) producers. 33.33% of the Pseudomonas isolates were found to be metallo-beta-lactamase (MBL) producers. Conclusion: This study shows that Klebsiella species, Coagulase Negative Staphylococci, Staphylococcus aureus and Non-fermenting gram negative bacilli were the commonest organisms associated with Neonatal sepsis. An alarmingly high rate of MRSA and ESBL producers was observed. The incidence and subsequent risk factors in the clinical management of newborn sepsis is a continuing major global public health concern. Although variety of sepsis screening methods available, blood culture and sensitivity is considered as the most promising test for diagnosis of neonatal septicemia because, the type of microorganisms associated and their antibiotic sensitivity pattern vary significantly. Antibiotic sensitivity testing of the isolates was performed in VITEK 2-compact (biomerieux). Results: From a total of 210 positive samples, the incidence of Gram negative bacilli predominates (65.2%) the Gram positive cocci (34.8%). Klebsiella species (94 out of 137) and coagulase negative Staphylococci (61 out of 73) are the most predominant bacteria isolated from neonatal sepsis. Antibiotic sensitivity pattern of the isolates varies significantly. However, most widely used carbapenems and cephalosporins demonstrated only marginal efficacy towards enteric GNB. Methods: This prospective study was conducted in the Department of Paediatrics, GITAM Institute of Medical sciences and Research, Visakhapatnam during January 2019 to December 2019. The institutional ethics committee had approved the study. Eighty clinically suspected cases of neonatal sepsis constituted material of study. All neonates who were clinically suspected sepsis, categorized into 0-72 h (early onset) or late onset (>3 days) sepsis based on day of presentation. Detailed history and clinical findings were recorded in the pro forma and screened with CRP and various hematological tests with predetermined cutoff values and at the same time blood culture was sent. Concerning a clinical scenario CSF, urine analysis, chest X-ray, and infective focus swabs were all taken. The length of treatment and time spent in the hospital were recorded. The death rate was calculated, and numerous risk variables were evaluated. Results: Intrapartum risk factors such as a prolonged rupture of membrane >24 h, a long labor, several filthy vaginal examinations (>3) before birth, and foul-smelling liquid were all linked to culture-proven newborn sepsis. Any of the two risk factors listed above was a good predictor of sepsis with a positive culture. Neonatal sepsis impacted a higher percentage of low birth weight and preterm babies. The sensitivity and negative predictive value of the I/T ratio and CRP were both high. The most prevalent isolates were Klebsiella species, followed by Staphylococcus aureus and Escherichia coli. While S. aureus was 80% sensitive to Cefotaxime and Amikacin, Klebsiella species and E. coli were only 20% sensitive. All were sensitive to Ofloxacin in range of 75-100%. Septicemia with multiorgan dysfunction was the most common mode of death with 100% risk of mortality. Conclusion: Intraparum risk factors are significantly with culture proven sepsis. All sepsis screening parameters used were statistically significantly associated with culture proven sepsis. I/T ratio and CRP were more sensitivity and had high negative predictive value.
