The paucity of immunoglobulins in preterm neonates consequently to the immaturity of immune system contributes to their high risk for systemic infection. The use of intravenous IgM enriched immunoglobulins, with higher antimicrobial activity than standard IgG, has been demonstrated in a retrospective study to reduce short term mortality in VLBW infant with proven sepsis. Larger, randomized prospective trials given the enormous burden of morbidity and mortality imposed by neonatal sepsis should urgently be addressed not only to validate this results but also to tailor consequences in terms of morbidity and mortality. This susceptibility to sepsis is related either to the immaturity of their immune system either to the exposure to invasive NICU maneuvers. The paucity of immunoglobulins contributes to the high risk of systemic infection. Very recent data on critical septic adults demonstrated that IgM enriched immunoglobulins reduce mortality. Available data about the role of IgM enriched immunoglobulins as adjuvant therapy in neonatal sepsis are still conflicting especially on VLBW infants that are the class of neonate more susceptible to infection. Furthermore, exceptionally preterm newborn children have lessened supplement components, polymorphonuclear chemotaxis and are obligated to debilitate their capacity pools. Goals and Goals: An attempt was made to conduct this planned study with accompanying goals to focus on administering IVIG in addition to antibiotics to improve the therapeutic consequences of sepsis in premature infants. Materials and Methods: Sixty preterm infants with sepsis were randomly assigned to study and control groups in the Neonatal Intensive Care Unit of Fauji Foundation Hospital, Rawalpindi for one-year duration from May 2019 to April 2020. The study group was given IVIG in addition to standard treatment. Results: A total of 60 patients were enrolled, 30 in the study and 30 in the control group. There were no differences between the sexes (male 50%, females 50%) of the included newborns, which is also visible in the study (men 47.7%, women 52.3%) and the control group (males 52.3%, females 47.7 %). Conclusion: The low level of immunity in premature babies causes increased morbidity and mortality in severe infections.
