The paucity of immunoglobulins in preterm neonates consequently to the immaturity of immune system contributes to their high risk for systemic infection. The use of intravenous IgM enriched immunoglobulins, with higher antimicrobial activity than standard IgG, has been demonstrated in a retrospective study to reduce short term mortality in VLBW infant with proven sepsis. Larger, randomized prospective trials given the enormous burden of morbidity and mortality imposed by neonatal sepsis should urgently be addressed not only to validate this results but also to tailor the optimal scheme of treatment.","author":[{"@context":"https://schema.org","@type":"Person","name":"FEDERICA Preterm infants have greater susceptibility to sepsis due to an immature immune system and lower immunoglobulin levels. Intravenous immunoglobulins (IVIG) have been used in several studies as an adjuvant treatment to improve this physiological immune deficiency, with different outcomes. METHODS: Very low birth weight (VLBW) infants who developed sepsis in the neonatal ICU were studied. They were randomly divided in 2 groups: one group was treated with antibiotics (Group I), and the other received antibiotics plus a 500 mg/kg/day of IVIG during 7 days (Group II). Serum IgG concentration was determined at initiation, during and after treatment Group I, and daily during the 7 days of therapy in Group II. RESULTS: The baseline IgG concentration in group II was 486 g/dL, and increased to 852 mg/dL after the first dose of IVIG (p</p><div class="ds-related-work--ctas"><button class="ds2-5-text-link Despite of advances in technology and optimal antibiotic treatment, incidence of neonatal sepsis and its complications remains unacceptably high especially in developing countries. Premature neonates in particular are at higher risk due to developmentally immature host defense mechanisms. Though not approved by Food and Drug Administration (FDA) U.S.A, off label use of intravenous immunoglobulin continues in many countries. Recent evidences showed no significant decrease in the mortality rate or other outcomes when intravenous immunoglobulin is administered in addition to standard therapies. consequences in terms of morbidity and mortality. This susceptibility to sepsis is related either to the immaturity of their immune system either to the exposure to invasive NICU maneuvers. The paucity of immunoglobulins contributes to the high risk of systemic infection. Very recent data on critical septic adults demonstrated that IgM enriched immunoglobulins reduce mortality. Available data about the role of IgM enriched immunoglobulins as adjuvant therapy in neonatal sepsis are still conflicting especially on VLBW infants that are the class of neonate more susceptible to infection. We provide a critical review on current literature.</p><div class="ds-related-work--ctas"><button class="ds2-5-text-link ds2-5-text-link--inline js-swp-download-button" data-signup-modal="{&quot;location&quot;:&quot;wsj-grid-card-download-pdf-modal&quot;,&quot;work_title&quot;:&quot;Adjuvant therapy in septic neonates with immunoglobulin preparationscontaining Ig isotypes in addition to IgG: A critical review of currentliterature&quot;,&quot;attachmentId&quot;:118958309,&quot;attachmentType&quot;:&quot;pdf&quot;,&quot;work_url&quot;:&quot;https://www.academia.edu/124799850/Adjuvant_therapy_in_septic_neonates_with_immunoglobulin_preparationscontaining_Ig_isotypes_in_addition_to_IgG_A_critical_review_of_currentliterature&quot;,&quot;alternativeTracking&quot;:true}"><span class="material-symbols-outlined" style="font-size: 18px" translate="no">download</span><span class="ds2-5-text-link__content">Download free PDF</span></button><a class="ds2-5-text-link ds2-5-text-link--inline js-wsj-grid-card-view-pdf" href="https://www.academia.edu/124799850/Adjuvant_therapy_in_septic_neonates_with_immunoglobulin_preparationscontaining_Ig_isotypes_in_addition_to_IgG_A_critical_review_of_currentliterature"><span class="ds2-5-text-link__content">View PDF</span><span class="material-symbols-outlined" style="font-size: 18px" translate="no">chevron_right</span></a></div></div><div class="ds-related-work--container js-wsj-grid-card" data-collection-position="9" data-entity-id="43962406" data-sort-order="default"><a class="ds-related-work--title js-wsj-grid-card-title ds2-5-body-md ds2-5-body-link" href="https://www.academia.edu/43962406/PHARMACEUTICAL_SCIENCES_SJIF_Impact_Factor_7_187_A_STUDY_TO_ASSESS_THE_IMPORTANCE_OF_INTRAVENOUS_IMMUNOGLOBULIN_IVIG_AS_AN_ADJUVANT_IN_THE_TREATMENT_OF_NEONATAL_SEPSIS_IN_PRETERM_BABIES_QR_code">PHARMACEUTICAL SCIENCES SJIF Impact Factor: 7.187 A STUDY TO ASSESS THE IMPORTANCE OF INTRAVENOUS IMMUNOGLOBULIN (IVIG) AS AN ADJUVANT IN THE TREATMENT OF NEONATAL SEPSIS IN PRETERM BABIES QR code</a><div class="ds-related-work--metadata"><a class="js-wsj-grid-card-author ds2-5-body-sm ds2-5-body-link" data-author-id="15402473" href="https://nagajunauniversity.academia.edu/Editoriajps">Editor iajps</a></div><p class="ds-related-work--abstract ds2-5-body-sm">Introduction: Newborn children conceived before 32 weeks of incubation are genuinely immune deficient with cord blood centralization of IgG being not as much as half contrasted with those found in infants conceived at full term. Furthermore, exceptionally preterm newborn children have lessened supplement components, polymorphonuclear chemotaxis and are obligated to debilitate their capacity pools. Goals and Goals: An attempt was made to conduct this planned study with accompanying goals to focus on administering IVIG in addition to antibiotics to improve the therapeutic consequences of sepsis in premature infants. Materials and Methods: Sixty preterm infants with sepsis were randomly assigned to study and control groups in the Neonatal Intensive Care Unit of Fauji Foundation Hospital, Rawalpindi for one-year duration from May 2019 to April 2020. The study group was given IVIG in addition to standard treatment. Results: A total of 60 patients were enrolled, 30 in the study and 30 in the control group. There were no differences between the sexes (male 50%, females 50%) of the included newborns, which is also visible in the study (men 47.7%, women 52.3%) and the control group (males 52.3%, females 47.7 %). Conclusion: The low level of immunity in premature babies causes increased morbidity and mortality in severe infections. Using IVIG along with antibiotics and other supportive therapies may improve outcome.</p><div class="ds-related-work--ctas"><button class="ds2-5-text-link ds2-5-text-link--inline js-swp-download-button" data-signup-modal="{&quot;location&quot;:&quot;wsj-grid-card-download-pdf-modal&quot;,&quot;work_title&quot;:&quot;PHARMACEUTICAL SCIENCES SJIF Impact Factor: 7.187 A STUDY TO ASSESS THE IMPORTANCE OF INTRAVENOUS IMMUNOGLOBULIN (IVIG) AS AN ADJUVANT IN THE TREATMENT OF NEONATAL SEPSIS IN PRETERM BABIES QR code&quot;,&quot;attachmentId&quot;:64289846,&quot;attachmentType&quot;:&quot;pdf&quot;,&quot;work_url&quot;:&quot;https://www.academia.edu/43962406/PHARMACEUTICAL_SCIENCES_SJIF_Impact_Factor_7_187_A_STUDY_TO_ASSESS_THE_IMPORTANCE_OF_INTRAVENOUS_IMMUNOGLOBULIN_IVIG_AS_AN_ADJUVANT_IN_THE_TREATMENT_OF_NEONATAL_SEPSIS_IN_PRETERM_BABIES_QR_code&quot;,&quot;alternativeTracking&quot;:true}"><span class="material-symbols-outlined" style="font-size: 18px" translate="no">download</span><span class="ds2-5-text-link__content">Download free PDF</span></button><a class="ds2-5-text-link ds2-5-text-link--inline js-wsj-grid-card-view-pdf" href="https://www.academia.edu/43962406/PHARMACEUTICAL_SCIENCES_SJIF_Impact_Factor_7_187_A_STUDY_TO_ASSESS_THE_IMPORTANCE_OF_INTRAVENOUS_IMMUNOGLOBULIN_IVIG_AS_AN_ADJUVANT_IN_THE_TREATMENT_OF_NEONATAL_SEPSIS_IN_PRETERM_BABIES_QR_code"><span class="ds2-5-text-link__content">View PDF</span><span class="material-symbols-outlined" style="font-size: 18px" translate="no">chevron_right</span></a></div></div></div></div><div class="ds-sticky-ctas--wrapper js-loswp-sticky-ctas hidden"><div class="ds-sticky-ctas--grid-container"><div class="ds-sticky-ctas--container"><button class="ds2-5-button js-swp-download-button" data-signup-modal="{&quot;location&quot;:&quot;continue-reading-button--sticky-ctas&quot;,&quot;attachmentId&quot;:93940870,&quot;attachmentType&quot;:&quot;pdf&quot;,&quot;workUrl&quot;:null}">See full PDF</button><button class="ds2-5-button ds2-5-button--secondary js-swp-download-button" data-signup-modal="{&quot;location&quot;:&quot;download-pdf-button--sticky-ctas&quot;,&quot;attachmentId&quot;:93940870,&quot;attachmentType&quot;:&quot;pdf&quot;,&quot;workUrl&quot;:null}"><span class="material-symbols-outlined" style="font-size: 20px" translate="no">download</span>Download PDF</button></div></div></div><div class="ds-below-fold--grid-container"><div class="ds-work--container js-loswp-embedded-document"><div class="attachment_preview" data-attachment="Attachment_93940870" style="display: none"><div class="js-scribd-document-container"><div class="scribd--document-loading js-scribd-document-loader" style="display: block;"><img alt="Loading..." src="//a.academia-assets.com/images/loaders/paper-load.gif" /><p>Loading Preview</p></div></div><div style="text-align: center;"><div class="scribd--no-preview-alert js-preview-unavailable"><p>Sorry, preview is currently unavailable.
