birth defects in the Iraqi city of Falluja has for the first time concluded that genetic damage could have been caused by weaponry used in US assaults that took place six years ago.

The research, which will be published next week, confirms earlier estimates revealed by the Guardian of a major, unexplained rise in cancers and chronic neural-tube, cardiac and skeletal defects in newborns. The authors found that malformations are close to 11 times higher than normal rates, and rose to unprecedented levels in the first half of this year – a period that had not been surveyed in earlier reports.

The findings, which will be published in the International Journal of Environmental Research and Public Health, come prior to a much-anticipated World Health Organisation study of Falluja’s genetic health. They follow two alarming earlier studies, one of which found a distortion in the sex ratio of newborns since the invasion of Iraq in 2003 – a 15% drop in births of boys.

hospital for children could not have opened in a needier location — since 1993, Basra province has seen a sharp rise in the incidence of childhood cancer.

"Leukaemia among children under 15 has increased by about four times," said Dr. Janan Hasan of the Basra Children’s Hospital.

<a title="20101022_child_cancer_patient_Basrah_childrens_hospital by Gorillas Guides, on Flickr" not have a high survival rate," she told AFP on the sidelines of the opening ceremony, where hapless parents with sick children in tow, many with the tell-tale baldness of chemotherapy, clustered around the paediatrician.

The facility, which was built with multinational assistance and funds, officially opened on Thursday but has been partially operational for several months, Hasan said.

"This hospital is a very important achievement, and I thank everyone who helped build it," she said.

"This is a very good effort, but we still do not have advanced equipment, labs and many medicines. We hope to acquire them over time."

Three-year-old Muntadhar, his green eyes staring dolefully from his mother’s arms, is one of the patients the hospital cannot help because it does not have the equipment.

Muntadhar, an intravenous insertion needle bandaged to his foot, was diagnosed with a cancerous tumour a year ago, his mother Inas Ahmed said.

"We took him to Iran a few months ago and they cut him open to try to help him," she said, lifting his orange Mickey Mouse T-shirt to reveal a deep scar across his belly.

"But they said they couldn’t help and now we are trying to take him abroad, maybe to Thailand," she said with a look of resignation. A hospital official said they were working through some charities to arrange the operation, which Muntadhar’s poor parents cannot afford.

Halima Mukhtar, draped in a loose black veil that showed her traditionally tattooed face and hands, carried her four-year-old son Musa on her right shoulder.

"Thank God for this hospital, otherwise I wouldn’t know where else to turn," she said, a bottle of medicine in one hand.

"In this world I have only God and the people at this hospital," she added," as Musa, wearing flip-flops on his feet and a surgeon’s mask that covered most of his face, stared down blankly from his perch.

Hasan said the increasing cases of cancer in Basra were best documented in a study published this year by the University of Washington in Seattle, conducted with input from her and the Ibn Ghazwan teaching hospital.

The report found leukaemia in Basra among children under 15 had grown year to year from 1993-2007.

"We observed 698 cases of childhood leukaemia between 1993 and 2007, ranging between 15 cases in the first year and 56 cases in the final year, reaching a peak of 97 cases in 2006," said the study, authored by Amy Hagopian and Tim Takaro, and published in the American Journal of Public Health.

"Basra’s childhood leukemia rates compare unfavourably to those of neighbouring Kuwait and nearby Oman, as well as the US and the European Union and other countries," the study said.

"The incidence of cancer is significantly higher in Basra province than in other parts of Iraq," said Mohammed Kamil, deputy director of the hospital.

Speculation has focused on industrial pollution, the huge volumes of burning gas from oil wells in the energy-rich province, and Basra’s position in the frontline of wars in past three decades: the 1980-1988 conflict with Iran, the 1991 Gulf War and the US-led invasion that overthrew dictator Saddam Hussein.

"We hypothesise that hazardous exposures during these wars may have been leukaemogenic," said the Hagopian-Takaro study.

What weapons were actually used in those wars, and by Saddam himself against his own people to put down a 1991 Shiite uprising in the south, is anyone’s guess.

Some of the conjecture has centred on depleted uranium weapons used by US and coalition forces in the 1991 war to liberate Kuwait and the 2003 US-led invasion.

But in Iraq the possible link between the radioactive metal and health problems has not been proven.

The hospital, decorated with portraits of Donald Duck and Mickey Mouse prancing about with his girlfriend Minnie, was built with help from the United States, Spain, Oman, the United Arab Emirates, the UN Development Programme and Project Hope.

"This project will have a capability to serve the people of Iraq and its people and children of Iraq for many years to come," said Brigadier-General Randal Dragon, Deputy Commander for Support of the US army base in Basra.

"From that perspective, all of us should be proud and hope this project will pay big dividends," he told AFP.

<a new US estimate of the number of Iraqis killed seven years after the US-led invasion serves as a reminder that civilians are dying on a daily basis in Iraq, writes Salah Hemeid

Former US secretary of state Madeleine Albright’s famous quotation apparently justifying the deaths of half a million Iraqi children as a result of the Washington- backed and UN-imposed sanctions against Iraq in the 1990s has often been remembered as a cold-blooded assertion of US policy objectives.
<div style="border-right: black 1px solid; padding-right: 5px; border-top: black 1px solid; Holding The Feet Of His Father Baquba Hospital Morgue October 18 2006.

The aphorism came to mind again last week when US media reported that the United States had finally released its first official compilation of data on Iraqi casualties, more than seven years after its invasion of the country.

The report, posted on the US Central Command website in July, drew little notice until last Thursday, when media outlets published details showing that 63,185 civilians and 13,754 members of the Iraqi security forces had been killed from early 2004 to August 2008.

It is not clear why the figures did not include casualties from the immediate aftermath of the US-led invasion in 2003, or from the period after August 2008. It is not clear either how the data were compiled and using what methodology.

The figures seem to represent a "policy engineered" anti-climax as the Obama administration, facing a mid- term election challenge, tries to bring an end to America’s misadventure in Iraq.

The number of Iraqis killed during the US-led invasion and its aftermath has long been hotly debated, estimates ranging from fewer than 100,000 to more than a million.

Knowing how these latest US figures were arrived at would speak volumes about how the United States is faring as it prepares to exit from Iraq.

The casualty figures released by Washington are lower than those from Iraqi government sources. Last year, the Iraqi Ministry of Human Rights reported that 85,694 Iraqis, including military and police personnel, had been killed from the beginning of 2004 through to October 2008.

In January 2008, the World Health Organisation (WHO) estimated that 151,000 deaths had taken place in the country due to the violence, with a 95 per cent confidence estimate of between 104,000 and 223,000 from March 2003 through to June 2006. The figures were based on the results of an Iraq family health survey published in the New England Journal of Medicine, a respected US journal.

Another estimate from the Iraq Body Count, a non- governmental organisation based in Britain that uses media accounts, has put the number of civilian dead in Iraq at 47,668 during the same period as the WHO study. The group’s latest figures for civilian deaths from violence in the country until September 19 2010 stood at between 98,252 and 107,235.

A 2006 survey in The Lancet, a British medical journal, estimated that more than 600,000 Iraqis had died as a result of the war, a figure more than 10 times higher than other estimates at the time.

Iraq has not officially reacted to any of the studies, though many Iraqis have rejected the new US figures on the number of civilian deaths in the conflict, saying that they are well below the actual numbers who have died.

The numbers are misleading, critics say, because they are not based on a well- defined methodology dealing with all violence-related deaths, including assassinations and in operations conducted by the US military.

Estimates of casualty figures during the US-led invasion and occupation of Iraq have been controversial because of the high political stakes involved and the possibility of manipulation aimed at swaying public opinion. The recent report was prompted by a Freedom of Information Act request from the National Security Archive at George Washington University.

Scepticism has arisen about these latest figures not only because of possible discrepancies and the mysterious standards used to establish the magnitude of the casualties, but also because the parties involved have been reluctant to tell the truth about this human tragedy.

A fundamental question is why the US military, which bears primary responsibility for the conflict, failed to address the issue start from the start and why it did not keep accurate records on the victims of the invasion and occupation.

The military’s apparent incapacity to provide statistics about the causalities of US air bombardments and other related operations is a real and pressing concern.

Another question of concern is why the US media, omnipresent in Iraq since the 2003 invasion, has not capitalised on its high standards of professionalism to gather accurate data about the human tragedy in Iraq.

The Associated Press kept a record for the period from 28 April 2005 to 30 September 2010 listing some 49,416 deaths.

Yet, even more disturbing than these US failures has been the failure by successive Iraqi governments to establish an efficient process of data collection to register the deaths of Iraqi citizens and to compensate their families.

Failure to collect data and dodgy statistics are not the only problems. There is also the problem of how to count deaths that are directly related to the war and occupation, separating them from deaths as a result of violence in the country.

Absent from the debate is any explanation of the humanitarian crisis that has struck Iraq since the 2003 US-led invasion, including increasing poverty, unemployment, the deterioration of health services and the destruction of the country’s ecological system.

Statistics such as those released by the US military have also largely ignored Iraqi fatalities caused by a lack of clean drinking water and a breakdown in utilities.

Humanitarian agencies like the International Committee of the Red Cross have warned that the country’s healthcare facilities face grave shortages of staff and supplies, with the water, sewage and electricity infrastructure being in critical condition.

Rates of cancer, leukemia and brain tumours, widely believed to have been caused by US weaponry, have been on the rise, some research suggesting that they rival those reported among survivors of the nuclear attacks on Hiroshima and Nagasaki.

The US military’s report on the death toll in Iraq comes at a time when US President Barack Obama has reached his lowest ratings in US opinion polls ahead of crucial mid-term elections next month.

The release of the statistics while Obama embarks on a campaign to drum up support for Democratic Party candidates cannot be a coincidence.

By publishing a limited number of casualties in Iraq, the Obama administration may be hoping that it can go ahead with its policy of "turning the page" in Iraq, ending the US military presence in the country by the end of next year.

Exiting from Iraq would benefit the Democratic Party, whose president vowed to end the legacy of the Republican Party and its president in Iraq.

If all goes to plan, Iraq will no longer be front-page news in America, as US soldiers pack up to leave in order to help Democrats achieve some sort of hoped-for victory in next month’s elections.

However, the very day this article was sent to print, a spate of bomb attacks across Iraq killed and wounded many people, serving as proof that the threat of death remains a part of daily life in the country.

If Albright’s idea that the price paid by Iraqi civilians for US policy "is worth it" can serve as any sort of reminder in this sad chapter of Iraq’s history, then it should be that the US-led invasion has turned into a humanitarian tragedy, as well as an American national predicament.

<a weapons linked to an upsurge in child deformity cases in Iraq, are being investigated by the Ministry of Defence.

The case raises serious questions about the UK’s role in the American-led offensive against the city of Fallujah in the autumn of 2004 where hundreds of Iraqis died. After the battle, in which it is alleged that a range of illegal weaponry was used, evidence has emerged of large numbers of children being born with severe birth defects.

Alastair Hay: It is vital America discloses what weapons were deployed

Alastair Hay is professor of environmental toxicology at the University of Leeds

There clearly is a problem in Fallujah as far as the health of newborn children is concerned. Doctors and residents in the city are convinced that many more children than usual are being born with deformities. So great is the concern that some doctors are apparently advising mothers living in the city not to have children. If this is indeed the case the problem could hardly be more serious.

Something has to be done to resolve the issue. It is far from clear if there is a real increase in birth deformities or whether it is a case of more cases being recorded. An answer to this could come by comparing records for births before and after the attacks on Fallujah. For this to be an effective approach full recording of all births would be needed over an extended period.

Another approach is that announced by the World Health Organisation (WHO) on 1 April which is to analyse the trend in birth defects in Fallujah and to compare it with other parts of Iraq. Again good records are required. The comparison may indicate there are problems elsewhere, but it may also find no disturbing trend.

What the WHO will be assessing will be the effects of war on a civilian population, and more particularly what effect a war has on birth outcomes. The investigation is unique and very welcome.

The details of how the investigation is to be conducted are not yet known. If the approach is just to compare trends in birth defects in different locations in Iraq it may not identify causes of these. A more detailed approach would be the logical next step. But if there is no evidence of deformities being greater than the norm why do a more searching inquiry?

But this is jumping the gun. Fallujah has been in the headlines because of the savagery of the fighting there in 2004. US forces used white phosphorus there in November of that year. Allegations have also been made that the US used depleted uranium munitions. It is essential the US says what was used by its forces.

I am not aware of any evidence to suggest white phosphorus or depleted uranium has been linked with birth deformities. However, a toxicologist’s reassurance will not be enough for residents in Fallujah and they deserve more. This WHO investigation will help to explain what effects war has on birth outcomes and that has to be a small comfort for clinicians and their patients in Fallujah.

Alastair Hay: It is vital America discloses what weapons were deployed – Commentators, Opinion – The Independent

Iraqi families who believe their children’s deformities are caused by the deployment of the weapons have now begun legal proceedings against the UK Government. They accuse the UK Government of breaching international law, war crimes and failing to intervene to prevent a war crime.

Lawyers for the Iraqis have sent a letter before action to the MoD asking the Government to disclose what it knows about the Army’s role in the offensive, the presence of prohibited weapons and the legal advice given to Tony Blair, Prime Minister at the time.

Legal actions against America are blocked by US federal immunity laws and the US government’s boycott of the International Criminal Court.

The offensive against Fallujah, codenamed Phantom Fury, in 2004 was described as the most bitter fighting experienced by American soldiers since the war in Vietnam. But US forces were assisted by British units.

On 21 October, British soldiers were ordered by the Cabinet to help US forces throw a "ring of steel" around Fallujah. Six days later, a British battle group of 850 troops made up of the armoured infantry from the 1st Battalion, The Black Watch, an armoured reconnaissance squadron from the Queen’s Dragoon Guards, elements of 40 Commando Royal Marines and supporting specialists including Royal Engineers and Royal Military Police were redeployed from Basra.

The battle group established a base at Camp Dogwood on the eastern approach to Fallujah where they provided essential aid and assistance to the subsequent attacks on the city.

Before the attack the former Attorney General, Lord Goldsmith is alleged to have warned Mr Blair about the legal dangers of committing British forces to the attack.

Public Interest Lawyers, the law firm representing the Iraqi families, wants the Government to release this advice in full and say whether any British soldiers were involved in the fighting or supplied or helped fire prohibited weapons. During the attack coalition forces are alleged to have used weapons including white phosphorus, a modern form of napalm, and depleted uranium.

The World Health Organisation has begun investigating evidence of a worrying rise in the incidence of birth defects in the city, which Iraqi doctors attribute to the use of chemical weapons during the battle.

Malak Hamdan, a British Iraqi researcher working with doctors in Fallujah, told The Independent: "Doctors in Fallujah are witnessing unprecedented numbers of birth defects, miscarriages and cancer cases. Now, according to gynaecologists, paediatricians and neurologists in Fallujah, the numbers of these cases have been increasing rapidly since 2005."

She explained that the most common birth defects involve the heart and the nervous system but there have also been reported cases of babies being born with two heads, upper and lower limb defects and eye abnormalities.

"What is more disturbing is that pregnant women are completely unaware that they are carrying an abnormal child until the day they give birth – traumatising the mother and the rest of the family," said Ms Malak.

Mazin Younis, a UK-based Iraqi human rights activist who visited the city before the attack, said: "When I visited Fallujah a few weeks before the attack, I was shocked to see the majority of people had not left the city. Many of them had no one to go to…. We attacked this city ruthlessly without any concern for the fate of tens of thousands of civilians who were still living there. The unlawful use of white phosphorus in built-up areas was… never objected to by the British Government who assisted in the attack on Fallujah."

Phil Shiner, the UK lawyer leading the legal challenge, said: "The rate and severity of both foetal abnormalities and inexplicable illnesses such as leukaemia or those suffered by our clients in infants born to mothers in Fallujah has been the subject of numerous reports and letters to governments…. Population data were obtained from a 1997 census and various subsequent estimates to calculate rates.

Results. We observed 698 cases of childhood leukemia between 1993 and 2007, ranging between 15 cases (2.6 per 100000 annual rate) in the first year and 56 cases (6.9 per 100000 annual rate) in the final year, reaching a peak of 97 cases in 2006 (12.2 per 100000 annual rate).

Conclusions. Childhood leukemia rates in Basrah more than doubled over a 15-year period. The test for trend was significant (P=.03). Basrah’s childhood leukemia rate compared unfavorably with neighboring Kuwait and nearby Oman, as well as the United States, the European Union, and other countries.

See also:

Medline Plus information about childhood leukaemia

World Health Organization information about depleted uranium

Nerve agents could be to blame for tripling of child leukaemia in Basra – Times Online

The authors, three of whom are from the University of Washington, say they hope their calculations can now pave the way for an investigation into reasons why the rates have climbed so high, and why they are higher than found in nearby Kuwait, or in the European Union or the United States.

The study documents 698 cases of leukemia for children aged 0-14 during the 15-year period, with a peak of 211 cases in 2006. Younger children had higher rates than older ones.

"By using a hospital cancer registry, we were able to measure a jump in leukemia rates from 3 per 100,000 youngsters in the first part of our study period, to a rate of almost 8 and a half in the final three years," said UW Department of Global Health faculty member Amy Hagopian, the paper’s lead author.

By comparison, Hagopian said, the European Union and the United States report rates of 4 and 5 per 100,000, respectively. She also noted Kuwait reports a rate of approximately 2 per 100,000 and Oman reports rates between 2 and 3, depending on the gender of the child (boys typically have higher rates, as do children from higher socio-economic classes).

"Studying childhood diseases in war situations is difficult," Hagopian noted. "Aside from the normal difficulties of controlling for referral patterns changes caused by war-time conditions, the political situation is also challenging. We were constantly worried about the political risks our medical colleagues were taking by collecting and reporting these data."

Another author of the paper, UW Department of Epidemiology Chairman Scott Davis, noted, "Another challenge was securing population data for purposes of calculating rates." He said census data were not collected after the U.S. invasion of Iraq and population patterns were thought to be disrupted by migration patterns, as well. Study authors say they used the most conservative assumptions available, so as not to overstate their findings.

The study was conceived by faculty at University of Washington and two Iraqi universities — Mustansiriya University in Baghdad and Basrah University—and the Fred Hutchinson Cancer Research Center in Seattle. The authors formed an ongoing partnership to support public health in Iraq after the 2003 U.S. invasion.

During the period studied, Basrah and its highly populated surrounding area, which includes farmland and oil fields, became a modern battlefield, pummeled by three consecutive wars, including the Iran-Iraq war in the 1980s, the first U.S. invasion in 1991and the second U.S. invasion in 2003.

The researchers now seek to understand the cause of Iraq’s increased rate of child leukemia by conducting a case-control study to compare children who got leukemia with those who did not. That sort of study allows researchers to see if there were differences in exposures between the cases and the controls. Some increased exposures related to child leukemia could include the byproducts of regional petroleum fires and benzene, which comes from gasoline sold by children at the side of the road as a result of disrupted fuel supplies, war-related nerve agents and pesticides, and the widespread use of depleted uranium munitions.

<a 14 October 2009 (IRIN) – In the late 1990s 22-year-old Manal Sabir Abdullah from Basra was diagnosed with lung cancer, from which she eventually died in 2004.

"Her cancer was bizarre as none of our relatives had cancer before and she had never had bad health or harmful habits," said her husband, Hassan Najim Ghanim. "None of the doctors could determine how she developed the disease but most believed it was probably caused by contaminated air, soil and water," he said.

Remnants from Iraq’s three recent wars – the Iran-Iraq war in the 1980s, the Gulf War in 1991 and the US-led invasion in 2003 – coupled with the absence of adequate government controls on emissions and industrial effluent, have turned Iraq into one of the world’s most contaminated countries, say officials.

"There are a number of environmental challenges in Iraq,” Environment Minister Narmin Othman told IRIN. “One of them is water, air and soil contamination caused mainly by emissions from cars and generators in crowded areas, unplanned use of chemical fertilizers, war remnants and bombing with depleted uranium."

She said her ministry had identified military vehicles and tanks contaminated with radioactive materials dating back to the wars of 1991 and 2003, but no action had been taken to get rid of them.

There was a lack of government supervision of the waste being discharged into the country’s two main rivers – the Tigris and Euphrates. This included waste from heavy industry, tanning and paint factories, as well as raw sewage and hospital waste, she said.

"The contamination levels are rising significantly in Iraq," she concluded.

Depleted uranium

The US-led coalition forces used depleted uranium (DU) as a “penetrator “ at the core of armour piercing tank rounds in the 1991 and 2003 wars. Amid growing reports of ill-health among veterans, an international campaign has sought a global ban on DU weapons on public health grounds.

The US Department of Defense has denied that depleted uranium is an exposure threat, but does monitor soldiers with embedded DU armour fragments as a result of combat operations. So far, the amounts of DU detected after tests “pose no known” health risk, William Winkenwerder, assistant secretary of defense for health affairs, said in a statement.

But in a landmark ruling in September 2009, a British jury concluded that exposure to depleted uranium in the 1991 Gulf War was the likely cause of the colon cancer that killed British veteran Stuart Dyson in June 2008.

DU is a heavy metal and a by-product of the uranium enrichment process. It can enter the human body by inhalation, eating contaminated food, eating with contaminated hands or by exposing an open wound to contaminated dust or debris, according to Rahim Hani Nasih, a doctor in Mosul.

It can also contaminate soil and water, and coat buildings with radioactive dust. Wind and sandstorms spread the contamination, leading to diseases, including cancer, Nasih said.

In a 2005 publication, the UN Environment Programme identified 311 sites in Iraq contaminated with DU and said cleaning them up would require several years. No figures were available from the Ministry of Health on how many cancer cases might have been related to or caused by contaminated war remnants.

Basra study

Qusai Abdul-Latif Aboud, head of the Enhancing Health Directorate (EHD – affiliated to the Health Ministry) in the southern governorate of Basra, said war remnants in Iraq had become one of the main causes of cancer – along with smoking, emissions of harmful gases, and other kinds of pollution.

An EHD study earlier this year had noted that 340 cases of leukaemia had been registered between 2001 and 2008 in Basra. This compares with 17 cases in 1988 and 93 cases in 1997, Aboud said.

The study focused only on leukaemia, as cases of the disease had risen sharply in Basra.

It also found that the amount of uranium in Basra’s soil had jumped from 60-70 becquerels per kilogram of soil prior to 1991 to 10,000 becquerels per kilogram in 2009. As much as 36,205 becquerels per kilogram have been recorded in areas with abandoned remnants of war.

He said EHD relied on the media and community leaders to spread awareness about self-protection and how to avoid contaminated areas.

sm/at/cb/oa

<a
