At the time, Carl Bosch, BASF’s Nobel Prize-winning engineer said, ‘The disaster was caused neither by carelessness nor human failure. Unknown natural factors that we are still unable to explain today have made a mockery of all our efforts. The purpose of the analyses is to improve understanding of accident causes and of ways to improve detection of major accident precursors. A bow-tie framework was developed in a collaborative process of engineers, psychologists and inspectors over many years. The nature of the model is that it combines technical, human and management components while preserving the temporal sequence of events of each accident scenario. economic operators who use during the production process hazard substances which can lead to major accidents with severe consequences regarding health, the safety of persons and of the environment. This article describes a part of the measures imposed by the European legislation to prevent, prepare and for the responsibility of such technologies owners in order to identify, evaluate the risks and limit the consequences of major accidents in which are involved hazard substances. In this context, managing chemical risk is especially important towards preventing both occupational accidents and major accidents. Directive 89/391/EEC and Directive 2012/18/EU, respectively, are enforced in the European Union (EU) to this end. These directives may be further complemented by the recent ISO 45001:2018 standard regarding occupational health and safety management systems. These three management systems are closely related.
