The Incident Declaration is a base for the analysis, follow up and corrective actions. Late declarations do not allow pertinent analysis. The safety videos may have been purged, the traces/elements/proof on site have been removed/altered and parties may have left.</p> <p>See also the <a href="https://admin-eguide.web.cern.ch/en/procedure/declaration-safety-incidents">admin e-guide Declaration of Safety Incidents</a>.</p> <p><strong>Occupational Accident</strong></p> <p>If your are injured and if the accident is deemed to be occupational in accordance with Paragraph 26 of <a href="https://cds.cern.ch/record/1504798/files/CERN_Circ_Admin_En_14_Rev3.pdf" target="_blank"><strong>Administrative Circular No. 14</strong> (e.g.</a> on site or while commuting), please fill also "<strong><a href="https://edh.cern.ch/Document/General/HS50">Declaration of Occupational Accident (HS50)</a></strong>". Remember to:</p> <ul> <li>Link the HS50 to the Incident Declaration of the event.</li> <li>Attach a <strong>medical certificate </strong>stating your injuries and the accident. You will find a <strong>template </strong>in the HS50.</li> <li>Submit your HS50 <strong>within 10 days</strong>. This delay is extended to 3 month if the victim is physically unable to make a declaration.</li> </ul> <p>A missing/incomplete HS50 may be prejudicial to the victim as an occupational accident may be denied this qualification resulting in less insurance coverage which can have important consequences in case of sequel.</p> <p><strong>Victim of a material accident with a CERN vehicle</strong></p> <p>If you are the victim of a material accident with a CERN vehicle, you must complete a "Constat European" in addition to the "<strong><a href="https://edh.cern.ch/Document/General/IncidentDeclaration">Incident Declaration</a>" </strong>mentioned above.</p> <p>If you are not responsible and you must be compensated, contact the CERN insurance manager whose name is also in the routing of the Incident Declaration form.</p> <p><strong>Victim of a material accident with a private vehicle</strong></p> <p>If you have an accident on CERN site that causes any damage to your vehicle and you think CERN may be liable for it, then make sure you call the fire bridge or the guards to establish a "Constat European".</p> <p>In the absence
