Burley also asked employees to approve a falsified home-visit document for Causey.</p><p>Causey lived in a southeast Dallas home with top Madison High School player Johnathan Turner, who was arrested in the killing and charged with murder. Causey’s mother, Tammy Simpson, told investigators that Burley warned her not to talk about the recruiting.</p><p>Efforts to reach Burley on Thursday were unsuccessful.</p><p>The investigative reports describe a sports culture at some DISD schools in which rules were ignored and students were fraudulently allowed to play on teams. When the report was completed this month, Superintendent Mike Miles took swift action, including firing athletic director Jeff Johnson and title-winning Madison basketball coach Roderick Johnson.</p><p>Miles has said the district will continue to look into student residency issues. DISD coaches have been accused of recruiting players before, even causing South Oak Cliff High School to forfeit its 2005 and 2006 boys state basketball titles.</p><p>Miles cited student safety concerns as the reason for the district’s crackdown. Causey and Turner lived on Cinnamon Oaks Drive without their family members. The house isn’t in the Madison attendance zone, and while it is within the Wilmer-Hutchins boundary, Causey had moved there from Richardson ISD. The University Interscholastic League prohibits students from playing for high schools outside their attendance zone.</p><p>Most of the fired employees were found to have approved residency documents without actually visiting the players’ homes. In one case, Madison coach Geary Walker fabricated a lease for a house near the school so his son, who attended DISD’s Townview Center magnet school, could play there, the report says.</p><p>When investigators visited the South Dallas house, the woman who answered the door said no students lived there. But when asked specifically about Walker’s son, she changed her story and showed them the room where she said Walker and his son stayed. It had cigarettes, an ashtray and a bottle of vodka — and no signs of the boy’s belongings.</p><p>Walker and his son actually live in a DeSoto house registered under his name, investigators found. He couldn’t prove he rented the South Dallas house, the report says.</p><p>Two other Madison coaches, Tracy Flentroy and Ronald Johnson, signed home-visit documents supporting that Walker’s son lived near Madison, the report says. But Johnson told investigators that he didn’t visit the house and only took Walker’s word.</p><p>Flentroy signed off on the form for Walker’s son, stating that there were pictures of the player in the living room.</p><p><em>The Dallas Morning News</em> uncovered questions this month about Flentroy and at least 13 Madison students who have had his South Dallas address listed as theirs in DISD records. Many, if not all, of the students were athletes who played for Madison, including Admon Gilder, a current player who is considered one of the best guards in the country.</p><p>Gilder’s family has a house in Far East Dallas, and DISD records show he attended middle school there. Gilder’s mother hung up the phone this month when asked about the South Dallas address.</p><p>The investigative report doesn’t mention the issue with Flentroy’s address. In an interview this month, Flentroy said he didn’t know why his address was listed in the students’ records.</p><p>DISD fired the 15 employees on June 6 — the last day of school. It included five athletic department administrators, who failed to oversee the process and make sure the residency documents were legitimate, district officials familiar with the case said.</p><p>The district said Thursday night that it didn’t have any investigative reports into the athletic officials. DISD spokesman André Riley wouldn’t explain why they weren’t investigated, citing personnel reasons.
