Currently, there is no approved treatment for the management of coronavirus disease (COVID-19). Drug repurposing of existing medications could be a possible way to find out a novel therapeutic entity to combat the COVID-19. Aim: To determine the clinical efficacy and safety of an Ayurveda intervention (Guduchighana Vati) in asymptomatic and mild-to-moderate cases of COVID-19. Materials and Methods: This was an open-label randomized controlled pilot study with a sample size of 30 participants (15 in each arm). The participants were asymptomatic or mild to moderate cases of COVID-19. Guduchighana Vati 500 mg twice daily for 10 days was administered in the study group and Hydroxychloroquine for 5 days in the control group. Paracetamol, Vitamin C, Multivitamin, and Zinc were also provided in the control group. The main outcome measures were to negative real-time reverse transcription-polymerase chain reaction (RT-PCR) assay for COVID-19, proportion of participants with negative RT-PCR for COVID-19 at 5 th and 10 th day, proportion of participants with clinical recovery, improvement in laboratory parameters, and incidence of adverse drug reaction/adverse event (ADR/AE). The results of RT-PCR and clinical recovery were compared between groups using Chi-square test. The data related to laboratory parameters were compared within group using paired sample t-test/Wilcoxon signed-rank test and between groups using independent sample t-test/Mann-Whitney test. Results: The proportion of participants with negative RT-PCR for COVID-19 in the Guduchighana Vati group (93.3%) was better as compared to the control group (66.6%) till 10 th day of the study period. Though, the results are statistically not significant (P = 0.068). All the symptomatic patients in the Guduchighana Vati group clinically recovered whereas one patient remained symptomatic in the control group on the 5 th day. No symptoms of COVID-19 were observed at 10 th day in both the groups. No ADR/serious adverse event were observed during the study period in either of the groups. In 30th March 2021 during the initiation of the study, there is no proven herbal add-on therapy 31 declared to combat COVID-19. Therefore, based on evidence from Pre-Clinical Studies, we 32 tested the safety and efficacy of polyherbal drugs from India’s one of traditional medical 33 systems i.e. Siddha Medical System namely “MAM Novel Siddha formulation” for the 34 management of COVID-19. These Studies’ outcomes will be useful to explore the 35 appropriate add-on Siddha drug in the management of COVID-19. 36 Methods: Objectives: Reducing the onset of clinical symptoms in asymptomatic patients, 37 Reduction in the duration of SARS CoV-2 viral infections and reduction of viral load from 38 the baseline were all the primary outcomes. The secondary outcomes were to assess 39 immunological markers, and safety profile and to document the profile of study participants 40 Analysis of an Open Labelled Randomized Controlled Trial to Evaluate the Efficacy and Safety of Siddha Supplement (MAM Granules) along with Standard of Care Treatment in the management of RT-PCR Positive Pre-symptomatic COVID-19 patients at COVID-19 Care Centre, Greater Noida, Uttar Pradesh, India. Section A-Research paper ISSN 2063-5346 3667 Eur. Chem. Bull. 2023, 12 (Si6), 3666 – 3678 based on the Siddha principles. It was an open labelled, single centre, double arm, 41 exploratory and comparative Randomized Controlled Trial. We recruited the RT-PCR 42 Positive tested for COVID-19 Patients at COVID Care Centre (CCC) situated at Govt. 43 Institute of Medical Sciences, Greater Noida, India. It was an exploratory study to evaluate 44 the efficacy of MAM granules in Pre - symptomatic COVID-19 patients, we planned to 45 recruit 32 participants in each group. A simple random allocation method was done. The 46 participants were assigned into 2 groups and allocated through randomization blocks in 47 Microsoft Excel in a 1:1 ratio in each group. During the time of enrolment, we collected data 48 on the history of illness, socio-demographics, and clinical examination. We did a systematic 49 clinical examination, determine adherence to interventions and assessment about any 50 complaints and adverse events at every visit. Statistical analysis was made using 51 standardized tests to compare the two interventional arms for both primary and secondary 52 efficacy measures. 53 Results: 54 The study group (SG) exhibited a significant result in converting RT-PCR Positive patients to 55 negative compared to the Control Group (CG). A statistically significant reduction in the 56 viral load of SARS-CoV-2 was documented in the SG compared to the CG. The Mean days in 57 relieving all these symptoms were significantly (p<0.001) lower in the SG (10.77±2.50) as 58 compared to the CG (34.19±6.03. The mean change in CRP, Ferritin and LDH levels in the SG 59 (1.74±1.54, 67.00±45.90, 155.99 ±78.90) was significantly (p<0.001) lower as compared to the CG 60 (5.80±5.63, 183.58±101.14, 320.82±104.09) D-Dimer (0.44±0.53, 0.99±1.57) was found to be non-61 significant (p>0.05) among the 2 groups. The mean SpO2 was significantly(p=0.001) (higher 62 in the SG (97.38±1.05) as compared to the CG (95.78±2.31). In both groups, there were no 63 adverse events (AEs) or serious adverse events (SAEs) reported until the completion of the 64 study. 65 Conclusions: 66 Siddha polyherbal formulation “MAM Granules” emerged as significantly effective and 67 efficacious in the management of patients with Pre-Symptomatic COVID-19 RT-PCR 68 Positive as compared to the Standard of Care. Two weeks of following the treatment 69 protocol also prevented the incidence of the patients becoming symptomatic of COVID-19. 70 Large-scale multicentric studies will strengthen these findings. 71 Trial Registration: This trial was registered in CTRI (the Clinical Trial Registry of India) 72 (Registration number - CTRI/2021/02/031420 on 19.02.2021). Ayurveda understands the patho-physiology from the perspective of deranged body constituents. Based on same principles and the clinical picture of COVID-19 worldwide, this opinion article is prepared. The information regarding disease progression, collated from authentic sources, the disease maybe a pitta condition primarily triggering kapha release. Pitta and Kapha together are instigating complications such as hepatitis, myocarditis, GI symptoms, pneumonitis, Disseminated intravascular coagulation (DIC) and Acute Respiratory Distress Syndrome (ARDS). Also, there happens to be a stark resemblance between advanced symptoms of COVID-19 with complications of Herpes zoster and ZSH (Zostersine-herpete) despite different viruses triggering that. As per Ayurvedic patho-physiology, herpes is a pitta predominant disorder and COVID-19 too seems to exhibit similar symptoms in advanced stages. Pitta pacifying medicines have been effective in conditions like herpes zoster, Chicken pox etc. Based on this information, a hypothesis about the management plan for COVID-19 is proposed in this opinion article.
