Aim and objective: This study was planned to explore the potential of AYUSH-64 as an add-on to conventional care in improving the clinical recovery and negative reverse transcription-polymerase chain reaction (RT-PCR) conversion in asymptomatic and mild COVID-19 cases. Materials and methods: An open-label randomized controlled study was conducted at Government Medical College, Nagpur, Maharashtra, India, with a sample size of 60 participants. In this study, asymptomatic or mild COVID-19 patients were randomized and allocated into intervention and control groups (CG) in a 1:1 ratio. AYUSH-64 two capsules (500 mg each) were administered thrice daily, after food with water for 30 days along with standard care in the intervention group (IG), while the CG received only standard care. The primary outcome was the proportion of participants who turned RT-PCR negative for COVID-19 at 7 th , 15 th , 22 nd and 30 th days. Secondary outcomes were the proportion of participants who attained clinical recovery at 7 th , 15 th , 22 nd and 30 th days, change in laboratory parameters on the 30 th day and incidence of adverse drug reactions/adverse events. The data were compared within group using paired sample t-test/Wilcoxon signed-rank test and between group using independent sample t-test/Mann-Whitney test. Results: Statistically significant difference was not observed in the proportion of participants who turned RT-PCR negative during each of the follow-ups (P = 0.134) and both groups demonstrated comparable efficacy. The clinical recovery in terms of complete relief in symptoms in the symptomatic participants was 60% and 37% on day 15 (P = 0.098) and 100% and 85.2% on day 30 (P = 0.112) in the intervention and CG, respectively. The improvement in the inflammatory markers such as interleukin (IL)-6, tumor necrosis factor-α (TNF-α), and D-dimer was statistically significant (P < 0.05) in the IG, whereas in the CG, it was statistically significant for D-dimer only. None of the participants developed any complications nor were any significant ADR/AE observed in the groups. Conclusions: In patients with asymptomatic and mild COVID-19, AYUSH-64, as add-on to standard conventional care, contributed to improved clinical recovery and demonstrated potential in reducing the levels of pro-inflammatory markers such as IL-6 and TNF-α. Since then the virus has spread to many countries and has become and pandemic. The mortality rate of the disease is very high but it varies from country to country depending on the medical facilities and the observance of isolation among the population. As the disease is very contagious many different treatment modalities have been tried. The therapeutic methods which are used are Ayurvedic and homeopathic along with that some Allopathic drugs are also used. Ayurvedic and homeopathy were found to be useful in asymptomatic patients rather than symptomatic patients. The drug which is used for treatment is antiviral drugs, protease inhibitors, antimalarial drugs, and neuraminidase inhibitors. But most promising results were obtained by using hydroxychloroquine, thus many countries are using this drug for treatment. The COVID-19 infection may be an asymptomatic, mild, moderate or severe illness with severe potentially lethal complications, such as severe pneumonia, respiratory failure, acute respiratory distress syndrome, sepsis, and septic shock due to induction of cytological storm. Diagnostic confirmation is by reverse transcription polymerase chain reaction. The treatment is mainly symptomatic with management of complications. Antiviral drugs like lopinavir administration have not been proven very useful and other drugs, such as Arbidol and favipiravir have some hope. Remdesivir, a drug approved by USFDA, has shown benefits in critical patients with COVID-19 infection. The COVID-19 pandemic, also known as the coronavirus pandemic, is an ongoing global pandemic of coronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Most people infected with the COVID-19 virus will experience mild to moderate respiratory illness and recover without requiring special treatment. According to the University of Washington's Institute for Health Metrics and Evaluation, "the number of cases that are being detected needs to be multiplied by 20 or more to get the number of infections that are occurring in India." The institute reportedly also projected that India will report over 1 million (10 lakh) deaths by the start of August. The cumulative death toll due to COVID-19 could rise to 6,65,000 by the end of July, with around 3,29,000 additional deaths between April 12 to August 1 2021. An ayurvedic approach towards the prevention and management; to avoid severe complications in early stage of infection would be beneficial as adjunct therapy. A better lifestyle, knowledge of food and nutrition, immune boosters, prophylactic medication and adjunct therapy can definitely help to the health system of India. Most of the time Ayurveda could not advice because of their metal substances so all the medical professionals can advice plant based compounds as precaution.
