Design: Participants received a Traditional Chinese Medicine assessment and followed an acupuncture protocol for 12 consecutive weeks. The acupuncture points used were Kidney 3, Governing Vessel 4, Urinary Bladder 23, with Heart 7 and Pericardium 6. Participants also completed a questionnaire package on a weekly basis. Outcomes measured: The questionnaire package consisted of self-report measures assessing symptoms of depression, anxiety, and various aspects of sexual function. Results: Significant improvement among male participants was noted in all areas of sexual functioning, as well as in both anxiety and depressive symptoms. Female participants reported a significant improvement in libido and lubrication and a nonsignificant trend toward improvement in several other areas of function. Outcomes measured: The questionnaire package consisted of self-report measures assessing symptoms of depression, anxiety, and various aspects of sexual function. Results: Significant improvement among male participants was noted in all areas of sexual functioning, as well as in both anxiety and depressive symptoms. Female participants reported a significant improvement in libido and lubrication and a nonsignificant trend toward improvement in several other areas of function. FSD is a common phenomenon affecting up to 26%-71% of the general female population worldwide. The umbrella term FSD subsumes four disorders: (1) hypoactive sexual desire disorder; (2) female sexual arousal disorder; (3) orgasmic disorder; and (4) sexual pain disorder. Objective: The aim of this pilot study was to assess immediate and longer-term effects of acupuncture quantitatively on symptoms related to female sexual dysfunction, using global-and specific-symptom instruments. Design: This was a time series study in a self-selected population of nonrandomized patients. Setting: The pilot study was conducted at a private women&#39;s health clinic in Reno, NV. Patients: Seventeen sequential subjects between ages 40 and 66 and clinically diagnosed with FSD were assessed at baseline and followed through time. Intervention: The intervention examined was a series of four acupuncture treatments. Patients were also administered five instruments to measure symptoms related to FSD before acupuncture treatment, just before the final treatment, and 3 weeks after the final treatment. Main Outcome Measures: Using internal controls, changes in symptoms over time were measured using multivariate analysis of variance for repeated measures. Results: Acupuncture promoted reductions in multidimensional symptoms related to FSD. Sexual desire improved as identified by two instruments, the Sexual Interest and Desire Inventory-Female (p = 0.01 for shortand long-term) and the desire subscale of the Female Sexual Function Index (short-term p = 0.002 and longterm p = 0.04). Psychological symptoms were also reduced, as measured by two separate instruments, the Greene Climacteric Scale (short-term p = 0.03, long-term p = 0.008) and the Menopause Rating Scale (shortterm p = 0.04, long-term p = 0.008). Acupuncture reduced anxiety in the short (p = 0.03) and long term (p = 0.01), reduced sexual dysfunction over the long-term (p = .03), improved urogenital symptoms over the short-term (p = .04) and somato-vegatative symptoms over the short (p = .01) and long-term (p = 0.03). Objective: To determine whether acupuncture is effective as a premature ejaculation (PE) treatment compared with paroxetine and placebo. Design, setting, and participants: The study was conducted with methodologic rigor based on Consolidated Standards of Reporting Trials (CONSORT) criteria. Ninety patients referred to the urology clinic at a tertiary training and research hospital with PE were included in this randomized controlled trial and randomly assigned into paroxetine, acupuncture, and placebo groups. Heterosexual, sexually active men aged between 28 and 50 yr were included. Men with other sexual disorders, including erectile dysfunction; with chronic psychiatric or systemic diseases; with alcohol or substance abuse; or who used any medications were excluded. Intervention: The medicated group received paroxetine 20 mg/d; the acupuncture or sham-acupuncture (placebo) groups were treated twice a week for 4 wk. Measurements: Intravaginal ejaculation latency times (IELTs) and the Premature Ejaculation Diagnostic Tool (PEDT) were used to assess PE. IELTs were calculated by using a partner-held stopwatch. Data were analyzed statistically. Results and limitations: Median PEDT scores of paroxetine, acupuncture, and placebo groups were 17.0, 16.0, and 15.5 before treatment, and 10.5, 11.0, and 16.0 after treatment, respectively ( p = 0.001, p = 0.001, and p = 0.314, respectively). Subscores after treatment were significantly lower than subscores before treatment in the paroxetine and acupuncture groups but remained the same in the placebo group. Significant differences were found between mean-rank IELTs of the paroxetine and placebo groups ( p = 0.001) and the acupuncture and placebo groups ( p = 0.001) after treatment. Increases of IELTs with paroxetine, acupuncture, and placebo acupuncture were 82.7, 65.7, and 33.1 s, respectively. Extent of ejaculation delay induced by paroxetine was significantly higher than that of acupuncture ( p = 0.001). The most important limitation of the study was the lack of follow-up. A total of 22 patients with pED were randomized into two groups. They were either treated with acupunture specific against ED (treatment group) or acupuncture specific against headache (placebo group). Nonresponders of the placebo group were crossed over to the treatment group. Prior to acupuncture, serum sexual hormone levels, IIEF score, nocturnal penile tumescence testing for three nights (Rigiscan) and the erectile response to 50 mg sildenafil were evaluated. Out of 21 patients, 20 completed the study, including 10 patients after crossover. A satisfactory response was achieved in 68.4% of the treatment group and in 9% of the placebo group (P ¼ 0.0017). Another 21.05% of the pateints had improved errections, that is, sufficient rigidity under simultaneous treatment with 50 gm sildenafil. Low frequency electrical stimulation (5 Hz and 10 mA) was applied to these four acupoints, whereas no stimulation was applied to the other four points. After 30 min, the electrical stimulation was terminated and all needles removed. Blood samples were drawn according to a ®xed time schedule, to study the pro®le of a number of stress hormones, for example, adrenocorticotropic hormone, antidiuretic hormone and cortisol, the gonadotrophines follicle stimulating hormone and leutinizing hormone, and the sex steroid testosterone and its binding globulin, within the treatment period. Based on a diary of both patient and partner, and an interview one month after the end of treatment, the changes of sexual activity were evaluated over a period of 12 weeks, starting from the four weeks prior to the treatment, the four weeks during the treatment period and the four weeks after the treatment. An improvement of the quality of erection was experienced by 15% of patients, while 31% reported an increase in their sexual activity. No changes in the pro®les of hormones were detected. The use of acupuncture as a mono-therapeutic modality in ED, did not in¯uence the pro®le of the stress and sex hormones, but did improve the quality of erection and restored the sexual activity with an overall effect of 39%. No de®nite conclusions can be drawn from this pilot study. Method: In a double-blind, randomized, and placebo-controlled clinical trial, a total of 60 male patients treated with an SSRI and suffering from MDD (mean age =32 years) and SSRI-I SD were randomly assigned to take either verum (R. damascena oil) or a placebo. Patients completed self-ratings of depression and sexual function at baseline, at 4 weeks later, and at the end of the study, 8 weeks after it started. Results: Over time, sexual dysfunction improved more in the verum group than in the control group. Improvements were observed in the verum group from week 4 to week 8. Self-rated symptoms of depression reduced over time in both groups, but did so more so in the verum group than in the control group. Conclusion: This double-blind, randomized, and placebo-controlled clinical trial showed that the administration of R. damascena oil ameliorates sexual dysfunction in male patients suffering from both MDD and SSRI-I SD. The sub-sample for this study (n = 3114) comprises participants who were receiving treatment with a SSRI or SNRI and did not meet the gender-specific criterion for global sexual dysfunction on the CSFQ. Results: Among this sub-sample, 95.6% of women and 97.9% of men exhibited impairment in at least one phase of sexual functioning. Men were significantly more likely than women to experience dysfunction in the desire phase (91.2% vs. 79.0%; OR = 2.76; 95% C.I. = 2.14 to 3.5) and the orgasmic phase (85.1% vs. 45.4%; OR = 6.9; 95% C.I. = 5.6 to 8.4) but were significantly less likely than women to experience dysfunction in the arousal phase (71.9% vs. 83.3%; OR = .51; 95% C.I. = .43 to .62). The prevalence of phase-specific dysfunction did not vary significantly by SSRI/SNRI for males or females. Conclusion: Among patients who do not experience clinically significant global sexual dysfunction on SSRI/SNRI monotherapy, dysfunction in at least one phase of the sexual response cycle is very common and may reduce sexual health-related quality of life. PSSD mainly consists of hypo-anesthesia of the genital area, loss of libido, and erectile dysfunction. Although different management options have been proposed, there is no consensus on the treatment for this syndrome. Herein we report on a young man affected by PSSD who regained sexual functioning after 3-month treatment with EDOVIS, a dietary supplement containing L-citrulline and other commonly used aphrodisiacs.
