We present the successful treatment of fulminant neonatal enteroviral myocarditis in a pair of monochorionic diamniotic twins with cardiopulmonary support, intravenous immunoglobulin and pocapavir, an enteroviral capsid inhibitor. The twins took an almost exact parallel hospital course, including day of extracorporeal membrane oxygenation (ECMO) cannulation, day of ECMO decannulation, improvement of cardiac function, discharge and status at follow-up. Newborns are at a higher risk of severe disease including meningoencephalitis, sepsis syndrome, cardiovascular collapse, or hepatitis. The mechanism of heart failure in patients with enterovirus infection remains unknown. Early diagnosis may help clinicians predict complications in those infants initially presenting with severe disease. An 11-day-old male newborn was admitted to our neonatal intensive care unit because of tachycardia and crises of cyanosis. His elder brother had febrile illness. The newborn was cyanotic, in respiratory distress, with tachycardia, low blood pressure and prolonged capillary refilling time. Limb pulse oximeter was around 85%. neonatal enterovirus infections are still controversial. To evaluate whether timing of IVIG administration might affect clinical outcomes of neonates with severe enteroviral infections. We retrospectively analyzed 67 neonates with culture-confirmed severe enteroviral infection, defined as hepatitis with coagulopathy and thrombocytopenia. Clinical features, outcomes and the usage of IVIG therapy were collected and analyzed. IVIG administered within 3 days of illness onset was classified as early IVIG therapy. Of the 67 cases, 38 (57%) were male, 27 (40%) were premature, 57 (85%) had disease onset within 7 days of life and all but 2 cases were caused by coxsackievirus B group. Like many other medical conditions, infection with SARS CoV-2 would cause complications during pregnancy and severe illness in pregnant women. Case Report: We reported a premature neonate (31 weeks gestational age, weight 1700 g) with subclinical myocarditis born to a mother with COVID-19. Her mother had no symptoms and confirmed infection with SARS CoV-2 during pregnancy. Four days after delivery, the mother was diagnosed with COVID-19 infection. The neonate had positive C-Reactive Protein (CRP), elevated cardiac enzymes, and lymphopenia but negative real-time Polymerase Chain Reaction assays (real-time PCR). Her subclinical myocarditis was treated with Intravenous Immunoglobulin (IVIG) and inotrope. We did not use antiviral therapy in medical treatment. The neonate was discharged 3 weeks after admission with normal cardiac biomarkers enzyme levels and cardiac function in echocardiography. Her follow-up chest x-ray after two weeks was also normal. Conclusion: It seems that subclinical myocarditis can be a complication of COVID-19 in neonates, and this infection could also cause preterm labor in infected pregnant women. The baby should be isolated from infected mother and from other neonates born to healthy mothers. Neonates born with critical and severe cardiac anomalies usually require lifesaving cardiac surgical intervention. The patient’s hemodynamics should be stabilized before surgery. The patient should be considered as a source of SARS-CoV- 2 transmission of infection even in the absence of any clinical, laboratory or radiologic evidence of COVID-2019. Medical personnel’s should use personal protective equipment (PPE) especially during intubation, extubation and handling patient secretions. The circuits of cardiopulmonary bypass should be small as possible to decrease the priming volume. This review is focused on anesthetic hemodynamic goals of different congenital heart disease and methods of protection of medical team in operating room and neonatal intensive care unit from transmission of SARS-CoV- 2 infection.
