Despite people in these vulnerable countries having done the least to contribute to the climate crisis—and being the least prepared for its impact—they are on the frontlines and reeling from its effects. More frequent and intense natural disasters and extreme weather are destroying livelihoods, intensifying conflicts, uprooting people from their homes, and exacerbating already dire humanitarian crises. In many of these countries, well over half the population depends on agriculture for their livelihoods. In these communities, climate disasters like droughts and floods reduce harvests and dismantle infrastructures already burdened by conflict."],"issues_with_the_current_approach":[0,"Current approaches are inadequate. Current humanitarian and climate financing is not reaching those who need it most. In fact, the more fragile a country is, the less climate finance it receives. However, only 20% of acutely malnourished children are accessing this treatment, despite its known effectiveness."],"issues_with_the_current_approach":[0,"The current system and infrastructure that exists to deliver treatment is complex, costly, and chronically underfunded. The current status quo unnecessarily divides children into moderate and acute malnutrition, which is served by two different UN agencies, using separate facilities, protocols, and supply chains. The unnecessary complexity has been costly and stunted scaling of treatment."],"our_solution":[0,"This simplified approach relies on a color-coded, numbered measuring tape to diagnose a child and a nutritional peanut paste known as ready-to-use therapeutic food (RUTF). Central to scaling this approach is the role of Community Health Workers (CHWs), able to bring this lifesaving care directly to affected communities. This approach is scalable, less expensive and proven to help more than 90 percent of malnourished children treated with it recover and have a chance at life. It has also proven it can be more cost-effective and efficient than many current approaches, with some estimates counting the simplified approach as costing 20% less per child treated and uses 25% less product than the traditional protocol. In a study of 27,800 children treated using the combined protocol in Mali, the IRC found that 92 percent of those treated recovered within weeks."],"shifting_the_status_quo":[0,"IRC has been researching simplified approaches for treatment of acute malnutrition for a decade and the body of research is now significant. The evidence is consistent that simplified approaches, including middle-upper-arm-circumference tab-based and simplified dosing, are more cost-effective ways to treat acute malnutrition with the same recovery rates compared to the original protocol. We therefore support the range of simplifications and adoption of all effective simplified approaches, including those in the WHO wasting treatment guidelines, that expand reach of treatment and/or result in reduced costs. people each year with impactful services, but there is still a large unmet need due to conflict, disaster, and the climate crisis. Understanding and improving the per unit cost of our programs can allow us to reach more people with high quality and impactful programs within limited budgets.&nbsp;\n\nThrough cost analysis, the IRC **doubled** its cost-efficiency in some program areas, enabling us to reach an estimated **400,000 additional clients**. We have codified the cost analysis methodology into **Dioptra, a software tool for rapid and consistent cost-efficiency analyses of humanitarian programs.**&nbsp;\n\nThe IRC leads a consortium of **nine** organizations with a combined **$6 billion** annual budget: Save the Children, Mercy Corps, Catholic Relief Services, Action Against Hunger, CARE, the Norwegian Refugee Council, the Danish Refugee Council, and FHI 360. We have also partnered with Microsoft Tech for Social Impact for their expertise in best-in-class technology. With the Dioptra tool, we are committed to generating and applying cost-efficiency lessons to our programs, enabling us to collectively reach millions more people with critical services.\n\n#### Dioptra\n\n**Dioptra simplifies the process of conducting cost analysis, helping humanitarian and development actors make use of limited resources effectively and efficiently.**\n\nDioptra is a software tool that uses program expenditure transactions and calculates the cost per output of program interventions. Critically, it leverages program staff’s firsthand knowledge of how programs were implemented, and how different expenses contributed to different programs. We estimate that this allowed us to expand programming and reach <strong>400,000</strong> more children with the same amount of spending.<strong>Cash</strong> - Using cost-efficiency data from more than 30 cash distribution programs globally, the IRC found that cash programs reaching more than 1,000 households were about two times more cost-efficient than those below that threshold, prompting new IRC guidance to reach at least 1,000 households to maximize the impact per dollar.&nbsp;<strong>Livelihoods</strong> - The IRC livelihoods program in East Africa, <u>Re:BUiLD</u>, uncovered that certain interventions enabled two to three times more refugees and host community members to achieve employment outcomes at the same cost compared to other interventions. While typical NGO cash disbursements are reactive and rely on emergency financial appeals, preventative measures are especially important as climate shocks continue to worsen globally and humanitarian funding needs remain unmet for both emergencies and preventative responses.&nbsp;\n\nThe IRC’s innovative pilot aims to catalyze a transformation in humanitarian preparedness for complex climate crises by disseminating real-time information about climate risks and distributing cash assistance *before* communities face disaster. This solution is based on extensive research demonstrating that cash and voucher assistance is an effective intervention to improve food security and avoid harmful coping practices during crises.\n\n&nbsp;\n\nDuring the 2022 agricultural season, our pilot delivered one-time lump-sum cash payments of N195,000 Naira (equivalent to US$450) to 1,450 households randomly assigned to one of two groups. Half of eligible households received the cash when triggered by the flood monitoring platform’s risk thresholds (e.g., when river water volume exceeded a certain amount and the probability of a flood was high). An equal number of comparable households (‘control group’) received equal cash payments after the flood hit. The intervention also included early warning messages to all households through community-based early warning workers. Approximately 20% of natural disasters are highly predictable, but[<u>less than 1%</u>](https://anticipatory-action-toolkit.unocha.org/) of overseas development assistance (ODA) is pre-arranged towards these efforts. In Jordan, a country hosting the second largest number of refugees per capita and still recovering from the COVID-19 pandemic, a significant portion of the population is affected by lasting distress. Surveys before the pandemic indicated that around 57.0% of refugees and 33.9% of Jordanians in urban areas were already experiencing lasting distress, including higher rates of depression, anxiety, and PTSD (IMC, 2019). However, some reports suggest that 94% of people with mental health conditions in Jordan are not receiving treatment.\n\n#### Why is finding a solution critical?&nbsp;\n\nUnmet mental health needs can spill out across society--negatively impacting employment, poverty, safety, educational attainment, family stability, healthcare systems, and more (Alegria et al, 2018). Barriers to accessing mental health services include a lack of awareness surrounding the need for care, stigma around mental health, and lack of trust in health services. As in many countries, mental health remains a taboo topic in Jordan, with significant cultural barriers to identifying challenges and seeking care. Urgent action is necessary to bridge these gaps and reach people with the necessary support. Research and innovation can enable mental health services to meet people where they are and put forward new, community-driven solutions.\n\n#### How was the solution developed?&nbsp;\n\nThe Airbel Impact Lab has developed a program called \"InforMH\", or “Yalla Nihki” in Arabic,&nbsp; to address the treatment gap in mental health services. Unlike other programs, the InforMH project takes an “ecosystem” approach, linking people in Jordan to various formal and informal existing services rather than duplicating those offerings, and complementing that with self-care practices and behavioral science strategies.&nbsp;\n\n![A person having a whatsapp conversation on an android phone](https://airbelimpactlabsa.blob.core.windows.net/airbel-asset-storage-container/informh.jpg)\n\nInforMH deploys encouraging outreach messages, backed by behavioral science, through digital and in-person platforms. The program starts by inviting people to take an online self-assessment to determine their mental health needs. Based on their results, people are provided with tailored recommendations to pursue either formal, informal, or self care. Evidence-based and behavioral science-informed messaging and reminders encourage participants to follow through with recommended care. The entire package of interventions is delivered using a Whatsapp chatbot. This helps support people along an ongoing journey, by reaching out to them with daily messages on platforms they already use (eg, WhatsApp), providing reminders, encouraging action and seeking help."]}],[0,{"_bookshop_name":[0,"projectVideo"],"embed_link":[0,"<iframe width=\"560\" height=\"315\" src=\"https://www.youtube.com/embed/su3a1eFT8vI?si=qGG804bZQL3Yq6M-\" title=\"YouTube video player\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" allowfullscreen></iframe>"]}],[0,{"_bookshop_name":[0,"rich-text"],"content":[0,"The program was co-designed and tested iteratively with hundreds of community members in Jordan over the past three years. For example, A/B testing was used to determine the most effective messaging to encourage people to engage with the program. Two versions of the website were developed, each with different framing woven throughout. Option A used messaging focussed on endorsement by respected experts and professionals, while Option B used messaging focussed on peer endorsement. Both professional and peer endorsement are backed by behavioral science evidence.&nbsp;\n\nWe reached over 400,00 people with this test on Facebook, of whom 3,700 clicks through to our website, which included mental health psycho-education content, self-care recommendations and a button to call a hotline. Peer endorsement led to 14% more unique visitors to the InforMH website and those who went to Website B spent 33% more time on it. Since Option B performed better at encouraging behavior change among the wider Jordanian population, we integrated this messaging, promoting peer endorsement by other relatable people in Jordan, throughout many areas of the program.\n\n![Two panels featuring findings from InforMH](https://airbelimpactlabsa.blob.core.windows.net/airbel-asset-storage-container/informh-2.jpg)\n\n### How will this solution be adapted/scaled for future use?\n\nFeedback from users has been positive, with participants finding the program personal, informative, and accessible. Many participants reported they felt cared for by the program, noting that they liked having the program check in on them everyday. However, there were some technical glitches&nbsp; and ongoing behavioral barriers that need to be addressed. In 2024, these learnings are being&nbsp; integrated into an improved program that will be delivered as a 3 month long “pilot study”, with approximately 400 people across the country to learn about how the program works in real world conditions. These next phases of this project will set us on a path to launch our intervention nationwide — with the potential to benefit 1.65 million people in Jordan with unmet mental health needs.
