Timely diagnosis and adequate treatment is crucial for managing this burning issue.Aims and Objectives: Early detection of neonatal septicaemia along with its infective etiological agent(s) and assessment of antimicrobial sensitivity pattern.Materials and Methods: After getting approval from institutional ethics committee, an institution based observational, epidemiological study was conducted in ninety two neonates, clinically suspected for sepsis, admitted in neonatal intensive care unit (NICU) were included in the study. Written consent was obtained from their guardian. Blood was drawn from the neonates for estimating routine parameters and cultured for isolation of causative agents using standard protocol. The bacteriological profile of neonatal sepsis is under constant change with the advances in the diagnosis and treatment. Blood culture is still the mainstay of investigation of potential sepsis in spite of the recent advances in molecular diagnosis of fungal and bacterial sepsis. Most common bacterial organisms responsible for neonatal septicaemia in developing countries like Bangladesh, India, Nigeria, Pakistan, are Klebsiella, Escherichia coli, Enterobacter etc. The present study was undertaken to describe the spectrum of the isolates in cases of neonatal septicaemia, and their antimicrobial susceptibility pattern. The study was carried out over a one year period. Method: A total of 357 suspected patients of neonatal septicaemia were included in the study. Blood samples were collected with aseptic precautions preferably prior to the initiation of empiric antibiotic therapy. The bottles were incubated aerobically at 37 0 C for 7 days and subcultured on blood agar and Mac Conkey agar. If growth appeared the isolates were processed as per standard microbiological techniques and the isolates were identified. Antibiotic sensitivity testing was performed on Mueller‑Hinton agar plates by modified Kirby‑Bauer disk diffusion method as per Clinical Laboratory Standard Institute guidelines. Results: Of the 357 samples studied, growth was observed in 154 samples. Gram positive organisms constituted 37.66% and gram negative bacilli constituted 62.34% of the isolates. The predominant pathogens isolated were Klebsiella species, Coagulase Negative Staphylococci, Staphylococcus aureus followed by Non fermenting gram negative bacilli. 71.43% of the Staphylococcus aureus isolates were found to be methicillin resistant.96.05% of the Enterobacteriaceae were found to be Extended Spectrum Beta Lactamase (ESBL) producers. 33.33% of the Pseudomonas isolates were found to be metallo-beta-lactamase (MBL) producers. Conclusion: This study shows that Klebsiella species, Coagulase Negative Staphylococci, Staphylococcus aureus and Non-fermenting gram negative bacilli were the commonest organisms associated with Neonatal sepsis. An alarmingly high rate of MRSA and ESBL producers was observed. For the effective management of neonatal sepsis, knowledge about causative agent and antibiotic sensitivity pattern play a vital role. Present study was undertaken to analyze the various risk factors of bacterial sepsis, to isolate aerobic bacteria causing neonatal septicemia and to study the antibiogram of bacterial isolates. Material and Methods: Study was done over a period of one year. 198 clinically suspected cases of neonatal septicemia were included in the study. Detailed record of various predisposing neonatal factors were noted in a pro-forma. Blood samples from the neonates were subjected to aerobic blood cultures. The identification of the causative organism was carried out by standard identification tests and antibiotic sensitivity testing was done as per Clinical and Laboratory Standards Institute guidelines. Results: Of 198 neonates studied, predisposing factors for neonatal sepsis were mainly low birth-weight and preterm. Blood cultures were positive in 128(64.64%) cases. Predominant organisms isolated were Staphylococcus aureus (54.7%) and Klebsiella spp. (14.1%). Among Staphylococcus aureus, 43(61.42%) isolates were MRSA and most of them were sensitive only to vancomycin and linezolid. Most of the MSSA were sensitive to cotrimoxazole, clindamycin, ciprofloxacin, vancomycin, linezolid. Most of the Gram negative bacilli were sensitive to imipenem, piperacillin-tazobactam and amikacin. It is estimated that up to 20% of the neonates develop sepsis and approximately 1% die of sepsis related causes. In India, sepsis is one of the commonest causes of neonatal mortality contributing to 19% of all neonatal deaths. The present study was undertaken to study aerobic bacterial profile of neonatal septicaemia by automated blood culture systems. MATERIALS AND METHODS This observational study was conducted in the Microbiology Laboratory of a tertiary care centre over a period of one and a half years [January 2013 to June 2014]. Blood samples of neonates of both sexes were collected and processed. Detailed history recording and clinical examination was conducted on each patient and findings were recorded. RESULTS Total 157 blood samples, 99 [63.05%] from male and 58 [36.94%] from female patients were processed. Out of 157 blood samples, 70 i.e. 44.58% were flagged as positive by BacT/ ALERT 3D 60 system and 87 [55.41%] were negative. Within the neonatal age group, 52 [74.28%] neonates with positive blood cultures were of the age less than one week. Out of 70 culture positive cases 64 [91.42%] deliveries were conducted in the hospitals, rest 6 [8.57%] were home-based deliveries. Out of 70, 12 [17.14%] neonates were having history of febrile illness in the mother during gestation. And 17 [24.28%] neonates were found to have history of Prolonged Rupture of Membranes [PROM] during delivery. 43 [61.42%] of the 70 positive cases had not attained their gestational maturity. Out of 70 blood culture positive neonates, 44 [62.85%] were having early-onset sepsis. Rest 26 [37.14%] were suffering from late-onset sepsis. All 70 blood cultures were monobacterial. Out of 70 isolates, 35 [50%] isolates were Gram-positive cocci and 35 [50%] isolates were Gram-negative bacilli. Least susceptibility of E. coli isolates was observed to Cefepime [18.75%] and Ampicillin [12.50%]. CONCLUSION Culture positivity was found more in male neonates as compared to females. The male-to-female ratio being 1.69: 1. The incidence and subsequent risk factors in the clinical management of newborn sepsis is a continuing major global public health concern. Although variety of sepsis screening methods available, blood culture and sensitivity is considered as the most promising test for diagnosis of neonatal septicemia because, the type of microorganisms associated and their antibiotic sensitivity pattern vary significantly. Materials and Methods: A retrospective analysis of blood cultures was carried out in the department of microbiology, SVS medical college, Mahabubnagar, Telangana for the period of June 2017 to October 2019. Blood cultures (in BacT/Alert, biomerieux) were incubated and growth was identified as per the standard protocols. Antibiotic sensitivity testing of the isolates was performed in VITEK 2-compact (biomerieux). Results: From a total of 210 positive samples, the incidence of Gram negative bacilli predominates (65.2%) the Gram positive cocci (34.8%). Klebsiella species (94 out of 137) and coagulase negative Staphylococci (61 out of 73) are the most predominant bacteria isolated from neonatal sepsis. Antibiotic sensitivity pattern of the isolates varies significantly. However, most widely used carbapenems and cephalosporins demonstrated only marginal efficacy towards enteric GNB. Methods Blood culture of all neonates who were suspected for septicemia was performed. Isolate identification and antimicrobial susceptibility testing was done by standard microbiological method. Results Out of the total 7,868 blood culture samples submitted to the microbiology laboratory, 1,572 (19.97 %) samples were from suspected cases of neonatal septicemia. The blood culture was positive for neonatal sepsis in 238 (15.13 %) samples. The incidence of gram positive and gram negative organisms were 44.1 % and 55.9 % respectively. Staphylococcus aureus remained the predominant isolate followed by Klebsiella spp. Most of the gram positive isolates exhibited higher resistance to penicillin and cephalosporin. Susceptibility to commonly used aminoglycosides and quinolones was found. Similarly, more than two third isolates of gram negative Enterobacteriaceae showed resistance to ampicillin.
