This action nullifies the registration of pain-management clinics when the department finds that the clinic fails to meet certain criteria. The practitioner is obligated to update his/her profile data.</p><p><strong>CLOSED</strong> &#8211; the status of a facility, establishment or education program that is no longer operating.</p><p><strong>CONDITIONAL ACTIVE</strong> &#8211; the licensing board or department imposed conditions on the practitioner&#8217;s license as specified in the final order. The licensed practitioner may practice his/her profession in the state of Florida under conditions specified by the licensing board or department. The practitioner is obligated to update his/her profile data.</p><p><strong>CONDITIONAL INACTIVE</strong> &#8211; the licensing board or department imposed conditions on the practitioner&#8217;s license as specified in the final order. The licensed practitioner is not authorized to practice in the state of Florida, but maintains a license. The practitioner is obligated to update his/her profile data.</p><p><strong>DELINQUENT</strong> &#8211; Chapter 456 F.S. &#8211; the licensed practitioner who held a CLEAR ACTIVE or CLEAR INACTIVE license, but failed to renew the license by the expiration date. The licensed practitioner is not authorized to practice in the state of Florida. The practitioner is obligated to update his/her profile data.</p><p><strong>DELINQUENT</strong> &#8211; Chapter 499 F.S. &#8211; this company may submit a renewal application for this permit within 60 days of the expiration date. Once a renewal application is received, the applicant&#8217;s permit will remain valid until a final decision is made by the department to deny or grant the renewal.</p><p><strong>RENEWAL APPLICATION RECEIVED</strong> &#8211; Chapter 499 F.S. &#8211; this company filed a renewal application within 60 days of the expiration date. The practitioner is no longer obligated to update his/her profile data.</p><p><strong>EMERGENCY RESTRICTION INACTIVE</strong> &#8211; the licensed practitioner is restricted by emergency order of the department. The licensed practitioner is not authorized to practice in the state of Florida, but maintains a license. The practitioner is obligated to update his/her profile data.</p><p><strong>EMERGENCY RESTRICTION ACTIVE</strong> &#8211; the licensed practitioner is restricted by emergency order of the department. The licensed practitioner may practice in Florida, but has restrictions specified by emergency order. The practitioner is obligated to update his/her profile data.</p><p><strong>EMERGENCY SUSPENSION</strong> &#8211; the licensed practitioner is suspended by emergency order of the department. Licensee may not practice in Florida while license is suspended under emergency order. The practitioner is obligated to update his/her profile data.</p><p><strong>EXECUTIVE ORDER ACTIVE</strong> &#8211; the practitioner is clear to temporarily practice nursing in the State of Florida in accordance with and for the duration of Office of The Governor Executive Order 10-06.</p><p><strong>EXECUTIVE ORDER EXPIRED</strong> &#8211; the practitioner is not authorized to practice because temporary authorization to practice nursing in the State of Florida under Office of The Governor Executive Order 10-06 has expired.</p><p><strong>EXPIRED (for Radiologic Technologists) </strong>&#8211; the licensed practitioner failed to renew their license by the expiration date. The practitioner is not authorized to practice in the state of Florida. A license that has been expired for less than 10 years may be reactivated. A license that is expired for more than 10 years automatically becomes null and void.</p><p><strong>MILITARY ACTIVE</strong> &#8211; the licensed practitioner, serving in the Armed Forces of the United States, is only authorized to practice in a military facility. Spouses of members of the Armed Forces are exempt from licensure renewal provisions in cases of absence from the state because of their spouses&#8217; duties with the Armed Forces.
