feedback, positive or negative and your comments will help to improve our services.</p> </div> </section> <section class="main-content"> <div class="container"> <h2>The feedback process<a id="theprocess"></a></h2> <p>Firstly we would encourage you to provide feedback as soon as possible. If you are raising a complaint, this has to be made within 6 months of the event or finding out you have reason to complain, but no longer than 12 months after the event itself. We may be able to look into events that happened more than 12 months ago, if there are good reasons why you could not raise the matter earlier.</p> <p>However, we recognise there may be occasions where you have concerns at the time care is being delivered and if so, we would encourage you to speak to the person or team responsible for your treatment, or the treatment of someone you care about. This would be for example the nurse in charge of a ward, the Senior Charge Nurse.</p> <p>If you are unable to do this or remain unhappy with the outcome, you can raise a complaint via the <a href="/get-involved/feedback-and-complaints/" title="Feedback and complaints">Patient Experience Team</a>. This Team can support you to raise a complaint in line with the <a href="/media/pphkbv9i/public-facing-chp.docx" title="Public Facing CHP">Complaint Handling Procedure</a>. For thank you cards or donations, please speak to the person in charge.</p> <p>You can also submit feedback, positive or negative, by completing our online feedback form <a href="#feedbackform" data-anchor="#feedbackform">below</a>. If you are unable to complete this form, have any communication needs, require documents in different formats, need independent advice or specific support in providing your feedback or making a complaint; you can <a href="#help" data-anchor="#help">find help below</a>.</p> <p>There are a number of services which <a href="/get-involved/feedback-and-complaints/independent-feedback-services/" title="Independent feedback services">manage feedback independently</a>.</p> <p>If you wish to provide anonymous feedback, you can do so by accessing the <a rel="noopener" href="https://www.careopinion.org.uk/tellyourstory" target="_blank">Care Opinion website</a>. This is a public website, run by an independent organisation, where patients and families can provide anonymous and honest feedback about their experiences of any of our health or care services within the last 3 years.
