</figure> 지난해 국내 매독(Syphilis) 환자 수가 역대 최대치를 기록했다. 매독 신고 체계 가동 이후 가장 많은 감염 수치다. 최근 미국·일본 등 해외에서의 매독 환자 증가세와도 연관성 있을 것으로 추정된다.<br /><br />16일 질병관리청의 감염병 통계 자료에 따르면 작년 한 해 연간 매독 환자는 2786명이었다. 10년 전(2014년 1015명)에 비해 3배 가까이 뛰었다. 해외에서 감염된 환자도 93명(3.3%) 나왔다. 미국은 매독 감염 건수가 2022년 기준 20만건을 웃돌아 1950년 이후 최다였고, 같은해 일본에서도 1만3000명 이상 감염된 것으로 집계됐다.<br /><br />기존 수치와 단순 비교하긴 어렵다. 질병관리청은 “2024년 매독 신고 건수 증가는 표본감시 체계에서 전수감시 체계로 변경돼 나타난 현상”이라고 설명한 바 있다. 신고 항목도 늘어나 기존에는 표본감시 기관에서 1~2기와 선천성 매독을 주1회 신고하던 게 모든 의료기관에서 1~3기 및 조기 잠복, 선천성 매독을 진단 후 24시간 이내 신고하는 것으로 바뀌었다.<br /><br />이를 감안해도 최근 매독 감염 증가세가 두드러진다. 전염성이 강한 대표적 성병인 매독은 감염 1기에서 보이는 피부 궤양의 형태가 매화 같다고 해서 붙여진 이름이다. 흔히 매독균이라 부르는 ‘트레포네마 팔리덤’(학명) 병원균 감염으로 발생한다.<br /><br />매독 1기는 균이 침범한 부위에 발생하는 통증 없는 궤양이 특징. 성기 부위, 질, 항문, 직장 등에 주로 생긴다. 특별한 치료 없이 자연적으로 호전되기도 한다. 2기로 넘어가면 가려움 없는 피부 발진이나 발열·인후통·피로·두통, 근육통 등이 나타난다. 3기는 장기와 중추신경계, 눈, 심장, 간, 뼈 등을 침범해 심각한 합병증을 유발한다. 감염 후 장기간 잠복해 수십년이 지나 3기 매독으로 발현되는 케이스도 있다.<br /><br />매독의 기원은 정확히 밝혀지지 않았지만 수간(짐승을 상대로 하는 변태적 성행위)을 통해 매독균이 사람에게 옮겨왔으며 콜럼버스의 신대륙 발견 이후 본격 전파, 15세기 유럽에서 광범위하게 퍼졌다는 설과 이미 구대륙에도 있었다는 가설로 나뉜다. <figure class="article-figure"> <div class="figure-img"> <img 성관계를 하는 인구집단이 매독균 감염에 더 큰 영향을 받는다고 분석했는데 과거 동성연애자로 알려진 교황 율리오 2세의 사인(死因)도 매독이었던 것으로 보인다. 유명 화가 빈센트 반 고흐의 정신착란 현상 또한 매독 후유증으로 짐작된다.<br /><br />매독은 한때 불치병이라 간주됐으나 20세기 중반 페니실린 치료제 개발로 환자 수가 급감했다. 하지만 최근 들어 매독 감염 사례가 늘어나는 추세다.<br /><br />국내에선 합병증 가운데 매독균이 눈을 침범해 실명에 이르는 사례가 상당수 발견된다. 국제 학술지 ‘성감염병(Sexually transmitted infections)’ 최신호에 실린 국내 연구진의 매독 환자 분석 결과를 보면 1.4%가 매독균 감염으로 눈에 합병증이 발생한 것으로 파악됐다.<br /><br />송수정 강북삼성병원 안과 교수, 김은아 창원삼성병원 안과 교수, 류수락 한양대 의예과 교수 공동 연구팀은 44만8085명에 달하는 건강보험심사평가원 매독 환자 빅데이터를 분석했다.<br /><br />가장 흔한 합병증은 포도막염으로 매독 진단 후 평균 2~3년 뒤 발병했다. 2010년 10만명당 0.18명이던 환자 수가 2019년에는 1.58명으로 9배가량 급증했다. 고혈압과 당뇨병, 이상지질혈증 등 대사성 질환이 있는 환자들은 매독성 포도막염 발생 위험이 1.5배 정도 높았다.<br /><br />포도막염은 눈을 감싼 포도막 조직에 염증이 생긴 것을 말한다. 포도막이란 망막과 공막의 중간층에 해당하는 막으로 홍채와 모양체, 맥락막을 통칭한다. 포도알처럼 보인다고 해서 붙은 이름. 매독성 포도막염이 심할 경우 시력을 잃기도 한다.<br /><br />특히 30대 남성(남성 매독 환자의 21.2%)과 20대 여성(여성 매독 환자의 18.2%) 감염 사례가 많았다. 송 교수는 “적절한 매독 치료를 받지 않으면 눈의 모든 부위를 침범해 심각한 염증을 일으킬 수 있다. 매독성 포도막염은 심할 경우 실명 위험이 높은 만큼 조기 발견과 감염
