Foi aplicado um questionário sobre a conduta adotada no diagnóstico e tratamento do dedo em gatilho. Resultados Foram analisados 243 participantes com média de idade de 37.46 anos, na maioria homens (88%), tempo de experiência de pelo menos 1 ano (55,6%), e da região Sudeste (68.3%). A análise dos questionários evidenciou que há consenso nos seguintes quesitos: diagnóstico somente com exame físico (73,3%), classificação de Quinnell modificada por Green (58,4%), tratamento inicial não cirúrgico (91,4%), infiltração de corticoide com anestésico (61,7%) tempo de tratamento não cirúrgico de 1 a 3 meses (52,3%), tratamento cirúrgico pela via aberta (84,4%), principalmente via Objetivo: a proposta deste estudo foi discutir o tratamento cirúrgico com os resultados, as complicações e as vantagens na utilização da internação hospital-dia. Método: no período de fevereiro de 2001 até janeiro de 2008, verificaram-se 25 crianças entre 3 meses e 6 anos, portadoras de polegar em gatilho congênito. Foram operadas 35 mãos, e 10 casos eram bilaterais. Observamos que a manifestação clínica quanto à dificuldade de extensão do polegar estava presente em todas as mãos operadas e nenhuma criança queixava-se de dor. As crianças foram submetidas à mesma técnica cirúrgica, anestesia geral inalatória, com incisão transversa na prega volar metacarpofalangeana do polegar acometido e abertura longitudinal da polia flexora A1. Não foi utilizado antibiótico e as crianças foram internadas pelo sistema hospital-dia. Resultados: Quinze crianças eram do sexo masculino e dez do sexo feminino. Dos pacientes com lesão bilateral, sete eram do sexo masculino e três do feminino. Quanto ao lado acometido, obtivemos sete polegares direitos, oito esquerdos e dez bilaterais. Como complicações, não houve nenhuma recidiva; um polegar evoluiu com infecção cutânea superficial e dois polegares apresentaram deiscência parcial da sutura após a retirada dos pontos. Conclusões: o polegar em gatilho congênito ou tenossinovite estenosante do polegar na criança pode ser tratado com segurança através da abertura simples da polia flexora A1, utilizando-se a internação hospital-dia. Abstract Introduction: The congenital trigger disorder or stenosing tenosynovitis of the thumb is the difficulty of extending the thumb, identified in early months of life. Objective: The purpose this study was to discuss the operative results, complications and advantages of the day hospital system. Como desfecho secundário, comparar as taxas de remissão da parestesia e dor residual entre as duas técnicas. Métodos De forma prospectiva, verificamos o aparecimento de dedo em gatilho e taxa de remissão da parestesia e dor no território do nervo mediano em série de pacientes adultos operados pela TA (n ¼ 34). Comparamos com coorte retrospectiva operada pela TE (n ¼ 33), pela mesma equipe de cirurgiões. A avaliação dos pacientes ocorreu por meio de questionário estruturado em consulta de retorno, com mínimo de 6 meses de pós-operatório. Resultados Sessenta e sete pacientes foram avaliados. Não houve diferença quanto ao aparecimento de dedo em gatilho (TA, 26,5% versus TE, 27,3%; p ¼ 0,94) e dor (TA, 76,5% versus TE, 84.8%; p ¼ 0,38). Os pacientes operados pela TA apresentaram menos queixas de parestesia do que os operados pela TE (TA 5,9% versus TE 24,2%; p ¼ 0,03). Conclusões Em nossa série, a técnica cirúrgica não influenciou o aparecimento de dedos em gatilho e dor residual. Five hundred and seven questionnaires were considered and the results show agreement in the following topics: classification, 78.5% uses the Gustilo-Anderson; surgical treatment indication, with the majority (76.3%) preferring to operate all kinds of fracture with manual irrigation (80.3%) and saline solution (85.4%); stabilization method, predominantly the external fixator, with 52.1%, 74.4%, 88.6%, and 89.0% for types II, IIIA, IIIB, and IIIC, respectively, and; primary closure indication, chosen by 74.2% for the type I. There was no consensus regarding the timing of soft tissues reconstruction and the duration of antibiotics treatment. Surgeons' opinions agreed with literature only for classification, surgical treatment indications, irrigation pressure, and indication for primary closure. A recessão gengival pode levar a queixas estéticas, sobretudo, quando acomete dentes ântero-superiores. Nestes casos, o recobrimento radicular é comumente o tratamento de escolha. Entretanto, alguns fatores podem interferir no prognóstico do tratamento, como, por exemplo, mal posicionamento dentário e erro na identificação da junção cementoesmalte comum nas lesões cervicais não-cariosas.O presente trabalho tem como objetivo relatar um caso clínico que ilustra recobrimento total de recessão gengival Classe I de Miller associada à lesão cervical não-cariosa localizada no dente 23, o qual apresentava vestibularização e extrusão. Paciente do sexo feminino, 36 anos, com queixa estética e de hipersensibilidade. Inicialmente, foi realizada lingualização do elemento dentário por meio de aparelho ortodôntico segmentado. A movimentação ortodôntica ativa durou 2 meses. Em seguida,foi conduzida cirurgia plástica periodontal pela técnica de retalho coronário associada a enxerto subepitelial retirado do palato. Três meses após a cirurgia, observou-se recobrimento total da recessão e simetria da margem gengival em relação ao dente contralateral. Realizouse, então, discreto desgaste na borda incisal do dente. Adicionalmente, a lesão cervical não-cariosa foi restaurada em resina composta. Estabilidade do resultado foi mantida após 2 anos de acompanhamento. Portanto, é possível concluir que procedimentos ortodônticos e restauradores podem otimizar o resultado estético da cirurgia plástica periodontal no tratamento de recessões gengivais em dentes com lesões cervicais não-cariosas e/ou mal posicionados. Ponseti Method is based on specific serial casting, Achillis tenotomy and use of abduction brace to maintain correction. This was different from standard treatment in Brazil at that time, based in many more months of serial casting and an extensive foot surgery. The Ponseti Method difusion is analized in a cronologic manner, with focus in the history of its difusion in Brazil and in the world, beginning in University of Iowa. Internet influence is discussed, as well as the Program-Ponseti Brasil‖ for training orthopaedic surgeons in Ponseti clubfoot treatment. Populational data were used to evaluate Ponseti Method difusion, before the Training Program. Four manners of direct evaluation of the impact of the Training Program are presented: the participants´reaction, enquiries about clinical practice with Ponseti Method after one year, evaluation of the impact of the Training Program in two cities, São Luis and Tesesina, and case series presentations of nine Brazilian Orthopaedic Clinics in a national panel in the Advanced Ponseti Course, two years after the Training Program‖Ponseti Brasil‖. To reinforce information about difusion, publications in developing countries are discussed, extending indications for use of the Ponseti Method. Tendencies of medical literature about clubfoot treatment were analized, through written english and latinoamerican databases, and proceedings of two national meetings Brazilian Orthopaedic Meeting and Brazilian Pediatric Orthopaedic Meeting were revised to evaluate presentations about clubfoot treatment (Ponseti and surgical). Difusion is also discussed in other countries, non governamental organizations, related to parents perspective, and related to costs of treatment.
