associated with increased risk of fragility fractures in both men and women, and osteoporosis is underdiagnosed in people with SMI.</p> <p>Severe mental illness (SMI) has been associated with reduced bone density and increased risk of fractures, although some studies have shown inconsistent results.</p> <p>This longitudinal population-based cohort study was conducted in UK and included 444 480 people. The aims were:</p> <ul class="wp-block-list"> <li>to estimate the incidence of recorded osteoporosis diagnosis and fragility fractures in people with SMI aged ≥50 years in the UK and</li> <li>to compare the incidence of recorded osteoporosis diagnosis and fragility fractures between people aged ≥50 years with SMI and those without, accounting for age, sex, social deprivation, smoking, alcohol, and body mass index (BMI).</li> </ul> <p>This study has demonstrated that a diagnosis of SMI is a risk factor for fragility fractures in both men and women, accounting for age, social deprivation, smoking, alcohol, and body mass index. The study data suggest that osteoporosis is underdiagnosed both in men and women with SMI (with a relatively more pronounced effect in women with SMI compared with those without SMI), as well as in men without SMI.</p> <p>According to the authors: ‘Further research is needed to explore inequalities in osteoporosis screening and treatment in the presence of SMI. Fracture risk assessment and appropriate osteoporosis treatment as indicated may need to be included in the annual comprehensive care plan in people with SMI. Advice on diet and resistance training to prevent osteoporosis and fractures should also be evaluated’.</p> <p>Read the article <a href="https://bjgp.org/content/early/2024/10/08/BJGP.2024.0055">here</a>!</p> <p>Integrated Rehabilitation Management in Older Adults With Hip Fractures can contribute not only to preventing refractures but also to improving postoperative outcomes and reducing overall socioeconomic costs.</p> <p>Standardized, comprehensive multidisciplinary rehabilitation has known beneficial effects on functioning, admission to nursing homes, costs, and mortality rates of patients following hip fracture. This study aimed to evaluate the clinical effectiveness of Fragility Fracture Integrated Rehabilitation Management (FIRM) in comparison to conventional rehabilitation after hip fracture surgery.</p> <p>In this trial, conducted in Korea, 203 hip fracture surgery patients aged 65 years and older were randomized and followed up for 1 year. Unlike conventional rehabilitation, which typically emphasizes basic exercises, FIRM takes a multidisciplinary approach, offering intensive, personalized care that includes PT, OT, functional movement training, and complication prevention. Based on standardized, comprehensive evaluations, FIRM also ensures ongoing care through early supported discharge and regional collaboration, facilitating continued treatment after discharge. The main outcome assessed ambulatory function, using the Koval and Functional Ambulation Category (FAC) scales.</p> <p>Patients of the FIRM group had greater improvements in Koval and FAC scores, along with most secondary outcomes (pain levels, quality of life, cognitive function, psychosocial factors, ADLs, mobility, depression, discharge to home, complications, and survival) during the 1-year follow-up. Moreover, the FIRM group showed higher rates of independent ambulation and recovery to prefracture ambulatory status than the conventional group.</p> <p>According to the authors: ‘<em>These findings provide valuable insights for the early postoperative care and management of patients with HF in South Korea and beyond.
