class="webcolor">Abstract</h3> <p><div><div style="text-align: justify;"><b>INTRODUCTION: </b></div><div style=""><div style="text-align: justify;"><span style="">Respiratory problems have been and are the leading causes of morbidity worldwide, among the diseases of the lower respiratory tract stand out, pneumonias which can mostly be acquired in the community, and some are acquired within hospitals. Pneumonias have predisposing factors such as overcrowding, socioeconomic level, nutritional status, among others. Of community-acquired pneumonias, the main causative agent is Streptoco pneumoniae in 50% associated with predisposing factors, as well as the pathogenicity and virulence of the agent, some cases of CAP are complicated by pleural effusions and some more severe cases with empyema that further complicate and put the lives of patients at risk as is the case I present Pneumonia complicated with empyema and pulmonary perforation due to s. pneumoniae in a school patient. </span></div><div style="text-align: justify;"><span style=""><br></span></div></div><div style="text-align: justify;"><b>OBJECTIVE:</b> A complicated clinical case of CAP is presented, reviewing risk factors and the importance of early diagnosis and appropriate management to avoid complications that put a patient's life at risk. <br><br></div><div style="text-align: justify;"><b>CLINICAL CASE:</b> We present the case of fem.de 7 years old, 15 days with cough, medical treatment is started without improvement, there is a worsening of the cough, fever is added, he goes to the doctor, treatment with antihistamine antibiotic with steroid and anitussive without improvement is started. He was admitted to the emergency room due to dyspnea, dry cough, chest pain, fever, auscultation, right hypoventilated pulmonary field and presence of bilateral basal fine rales, radiologically found right pleural effusion with deviation of structures of the mediastinum and condensations in the left pulmonary field. Thoracic USG was requested, confirming purulent right pleural effusion and pericardial effusion. A pleural tube was placed, a few minutes later it began with facial edema, respiratory difficulty and the presence of rapidly progressive generalized emphysema covering up to upper extremities, thoracotomy was performed and the middle lobe was found with data of hepatization, necrosis and perforation in the posterior portion, performing lobectomy. </div><div style="text-align: justify;">Pleural fluid culture: Streptococcus pneumoniae was isolated, gram stain with gram-negative Diplococos, BARR negative, favorable evolution was removed chest tube 7 days later with favorable evolution without restrictions and with saturations without oxygen of 93 to 95 %it was managed with triple antimicrobial scheme.
