This model of care has been reported to be associated with a significantly lower probability of subsequent fractures and mortality rates.</p> <p>The aim of this prospective cohort study, conducted in Spain, was to analyze the effect of the implementation of a FLS model of care over the survival and mortality rates following a hip fracture. A total of 1366 60+ patients were included in this study (353 before FLS implementation and 1013 after FLS implementation). </p> <p>The study showed the following results:</p> <ul class="wp-block-list"> <li>Anti-osteoporotic drugs were more frequently prescribed after FLS implementation and there was an increase in adherence to treatment.</li> <li>Patients after the implementation of the FLS protocol had a lower all cause one-year mortality and a decreased risk of suffering a second osteoporotic fracture.</li> <li>The implementation of a FLS model of care reduced surgical-related complications (i.e. infection, cut-out, cut-in, aseptic loosening, non-union, dislocation and medical complication). These results highlight the potential cost-effectiveness of this model of care.</li> </ul> <p>According to the authors: ‘<em>The continuation of further studies, with longer follow-up periods, would be recommended to evaluate the long-term mortality and second fracture rates of the FLS protocols on hip fracture patients.</em>’</p> <div class="wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex"> <div class="wp-block-button is-style-fill"><a class="wp-block-button__link has-accent-3-background-color has-background wp-element-button" href="https://link.springer.com/article/10.1007/s40520-024-02761-3">Read the article here!</a></div> </div> <p></p> <p>A recent study identified several significant independent prognostic risk factors for the development of Prosthetic Joint Infections (PJI) after hemiarthroplasty, including BMI, operation time and experience level of the surgeon.</p> <p>The primary aim of this study was to identify risk factors and validate earlier reported risk factors for PJI after hemiarthroplasty. The secondary aim was to assess peri‑operative clinical outcomes, adverse events and mortality rates in PJI patients after hemiarthroplasty. 2044 patients were included in this observational cohort study, of which 72 patients developed PJI, according the MSIS criteria, after hemiarthroplasty.</p> <p>The results showed that:</p> <ul class="wp-block-list"> <li>Body Mass Index (BMI) >30, operating time of <45 min, occurrence of haematoma, decreasing level of hemoglobin and re-operation for luxation were significant independent prognostic risk factors for development of PJI after hemiarthroplasty. Diabetes Mellitus and >20 hemiarthroplasties performed by the surgeon in the previous year were prognostic protective factors.</li> <li>The all-cause mortality in patients with PJI one year postoperatively was 40%.</li> <li>The all-cause mortality was higher and the in-hospital stay was longer in patients with a PJI compared to patients with no PJI.</li> </ul> <p>According to the authors: ‘<em>A PJI is one of the most feared complications after hemiarthroplasty of the hip, causing high rates of morbidity and mortality. To enable preventive measures against PJI, further determination and validation of risk factors for PJI after hemiarthroplasty are needed.</em>’</p> <p>Read the article <a href="https://www.injuryjournal.com/article/S0020-1383(23)00912-9/fulltext">here</a>!</p> <p>Preoperative frailty increases the risk of postoperative delirium in older patients undergoing hip fracture surgery. </p> <p>Postoperative delirium (POD) is a prevalent complication in older patients with hip fractures undergoing surgery. In this study, conducted in China, 148 older patients who underwent hip fracture surgery were enrolled to investigate the association between preoperative frailty and the risk of POD. Delirium was evaluated postoperatively on days 1, 2, 3, 5, and 7 using the CAM scale, whilst frailty was assessed based on the Frailty Index (FI).</p> <p>The results of the study showed that the overall incidence of POD on day 7 was 24.3%, and preoperative frailty was associated with a significantly higher risk of POD compared with non-frailty. Over 400 attendees from various disciplines and countries in the region were welcomed by Malaysia's warm hospitality. Plenary discussions also focused on better nutrition pathways and targeted mobilization through physiotherapy. Two new national FFNs, FFN Singapore and FFN Chinese Taipei, were introduced, and Indonesia expressed intent to form one. Additionally, FFN Global’s president-elect, Karen Hertz, rallied the nursing community and allied health practitioners to meet the challenges of an aging region.
