Since 2017 our surgery program has used defined protocols and educational curricula pre- and post-surgery. In response to patient outcomes, our educational content was refined to include quizzes to assess patient knowledge and surgical preparedness. We aim to optimize adolescent pre-bariatric surgery preparedness by improving overall aggregate pre-surgical assessment performance from 68% to 80% within 12 months. Methods: A multidisciplinary improvement team was developed within the weight management clinic (WMC) of our tertiary care, free-standing children’s hospital. A manual has been utilized since 2017, with limitations in consistent delivery and patient uptake of information. The curriculum has been improved to include quizzes administered during WMC visits prior to bariatric surgery. The initial outcome measure is the pre-surgical quiz score of adolescents preparing for bariatric surgery. Process measure was the number of questions answered correctly to test the questions. Baseline performance was determined by a patient assessment survey of pre-surgical preparedness at patient visits. Plan-Do-Study-Act cycles (PDSA) included: 1) creation and implementation of a refined curriculum, 2) development of 5 new quizzes based upon learning objectives, and 3) improving provider-lead teaching and quiz administration within clinic workflow. Run charts assessed impact over time. Results: A total of 346 quiz questions were administered to 34 adolescents. The outcome measure improved from a baseline mean of 68% to 86% following PDSA 2 cycles, and it was sustained. Conclusion/Implication: Patient/family comprehension of surgical preparedness improved with standardized education via team member-led teaching and assessment using quizzes during pre-surgical clinic visits. Clark</a> </p> <p class="card-text"><strong>Abstract:</strong></p> Background: Adolescents with obesity are at higher risk of disordered eating than the general population. Detection of eating disorders (ED) is difficult. Screening questionnaires may aid in early detection of ED. Our team’s prior efforts focused on increasing ED screening rates to ≥90% using a validated 10-question adolescent binge eating disorder screening questionnaire (ADO-BED). This aim was achieved. We then aimed to improve treatment plan initiation of patients ≥12 years of age who screen positive for BED within our WMC from 33% to 70% within 12 months. Methods: Our WMC is within a tertiary-care, free-standing children’s hospital. A3, an improvement framework, was used. A multidisciplinary team (physicians, nurses, registered dietitians, psychologists, and exercise physiologists) was created. The outcome measure was documentation of treatment plan initiation of those who screen positive (goal 70%). The process measure was ADO-BED screening rate of WMC patients (goal ≥90%). Plan-Do-Study-Act (PDSA) cycle 1 included provider education on current literature and treatment plan initiation based upon ADO-BED responses. PDSA 2 involved increasing documentation of treatment plan and retrain process to providers. Pre-defined treatment plans were: 1) repeat screen in 3-6 months, 2) resources provided only, or 3) comprehensive multidisciplinary weight management team evaluation. Run charts monitored impact over time. Results: Within 9 months, 166 patients were seen in WMC. Process measure showed sustained performance above goal (mean 98%). Outcome measure showed special cause improvement from mean of 33% to 100% (n=31). Of treatment plans provided, 45% received Plan 1, 4% Plan 2, and 46% Plan 3. Conclusion: Through a multidisciplinary improvement team approach, we maintained sustained ADO-BED screening performance, and, prior to our 12-month timeline, achieved our project aim. Our efforts may serve as a model for other multidisciplinary WMCs. The analysis methodology will be based on a systematic presentation of the existing advances in the subject under a qualitative approach, in which the results are detailed. We hold that an important part of the Latin American countries that used to be the emitters of migrants have had to generate, with greater or lesser success both nationally and regionally, ordinary and extraordinary migration regulatory tools to respond to the rapid intensification of the current Venezuelan migratory flows. This fact beyond implementing policies for the reception and integration of this population marks a new moment that represents a huge challenge both for the receiving States and for the young Ibero-American institutional migration system. While research on the ecological impacts of LWD has been conducted in temperate streams, LWD's influence on tropical stream biodiversity remains understudied, making this investigation particularly valuable for future conservation efforts. The Sura River in La Selva Biological Station includes both LWD and open channel sites. We sampled paired LWD and open-channel sites using minnow traps, Promar traps, and dip nets. Vertebrates were identified as species, while macroinvertebrates were identified to order level. We quantified abundance, richness, and Shannon diversity at each. We captured a total of 467 individuals, including 2 turtles, 17 fishes, 1 freshwater crab, 39 shrimp, and 408 other macroinvertebrates. Total abundance was significantly higher in LWD sites. Species richness was marginally higher in LWD sites, but the Shannon diversity index did not differ significantly with habitat. Shrimp (Macrobrachium olfersi) length was significantly higher in LWD areas. Increased food resources and microhabitat availability could contribute to higher abundance, richness, and organismal size in LWD environments. Through a comprehensive literature review, we found consistent evidence supporting the positive influence of Transcendental Mindfulness Meditation on reducing anxiety, enhancing overall well-being, and decreasing perceived stress levels within this demographic. The mechanisms underlying these effects include heightened self-awareness, improved emotional regulation, and the development of effective stress-coping strategies. These findings have significant implications for mental health interventions targeting young females. However, the reviewed studies had some limitations, such as small sample sizes and reliance on self-report measures. To advance this field, future research should focus on larger sample sizes and utilize a broader range of measurement methods, including neuroscience assessments. Additionally, investigating the temporal relationships between Transcendental Mindfulness Meditation, proposed mediators, and anxiety symptoms will help establish causal specificity and a deeper understanding of the precise mechanisms of action. The development of integrative models based on these mechanisms can further enhance the effectiveness of Transcendental Mindfulness Meditation as an intervention for anxiety in this demographic. These properties are obtained through experimental tests conducted on prisms or wallettes and depend on various factors, with the HCB contributing significantly to overall strength. One influential factor in the compressive behaviour of masonry is the thickness and method of mortar placement. Mexican regulations stipulate mortar placement over the entire net area (full-shell) for strength computation based on the gross area. However, in professional practice, there's a growing trend to place mortar solely on the lateral faces. Conversely, the United States of America standard dictates mortar placement and computation over the net area of HCB. The Canadian standard specifies mortar placement solely on the lateral face (Face-Shell-Bedding), where computation necessitates the use of the effective load area, corresponding to the mortar's placement area. This research aims to evaluate the influence of different mortar placement methods on the axial compression behaviour of HCBM. To achieve this, an experimental campaign was conducted, including: (1) 10 HCB specimens with mortar on the entire net area, (2) 10 HCB specimens with mortar placed on the lateral faces, (3) 10 prisms of 2-course HCB under axial compression with mortar in full-shell, (4) 10 prisms of 2-course HCB under axial compression with mortar in face-shell-bedding, (5) 10 prisms of 3-course HCB under axial compression with mortar in full-shell, (6) 10 prisms of 3-course HCB under axial compression with mortar in face-shell-bedding, (7) 10 prisms of 4-course HCB under axial compression with mortar in full-shell, and, (8) 10 prisms of 4-course HCB under axial compression with mortar in face-shell-bedding. A combination of sulphur and fly ash in a 2:1 ratio was used for the capping material, meeting the average compressive strength requirement of over 35 MPa as per NMX-C-036 standards. Additionally, a mortar with a strength of over 17 MPa was utilized for the prisms. The results indicate that prisms with mortar placed over the full-shell exhibit higher strength compared to those with mortar over the face-shell-bedding.
