class="ds-related-work--abstract ds2-5-body-sm">Background: Sleep apnea (OSA) is a common sleep disorder characterized by recurring breathing pauses during sleep caused by a blockage of the upper airway (UA). The altered UA structure or function in OSA speakers has led to hypothesize the automatic analysis of speech for OSA assessment. In this paper we critically review several approaches using speech analysis and machine learning techniques for OSA detection, and discuss the limitations that can arise when using machine learning techniques for diagnostic applications. Methods: A large speech database including 426 male Spanish speakers suspected to suffer OSA and derived to a sleep disorders unit was used to study the clinical validity of several proposals using machine learning techniques to predict the apnea-hypopnea index (AHI) or classify individuals according to their OSA severity. AHI describes the severity of patients' condition. We first evaluate AHI prediction using state-of-theart speaker recognition technologies: speech spectral information is modelled using supervectors or i-vectors techniques, and AHI is predicted through support vector regression (SVR). Using the same database we then critically review several OSA classification approaches previously proposed. The influence and possible interference of other clinical variables or characteristics available for our OSA population: age, height, weight, body mass index, and cervical perimeter, are also studied. Results: The poor results obtained when estimating AHI using supervectors or i-vectors followed by SVR contrast with the positive results reported by previous research. This fact prompted us to a careful review of these approaches, also testing some reported results over our database. Several methodological limitations and deficiencies were detected that may have led to overoptimistic results. Conclusion: The methodological deficiencies observed after critically reviewing previous research can be relevant examples of potential pitfalls when using machine learning techniques for diagnostic applications. We have found two common limitations that can explain the likelihood of false discovery in previous research: (1) the use of prediction models derived from sources, such as speech, which are also correlated with other patient characteristics (age, height, sex,…) that act as confounding factors; and (2) overfitting of feature selection and validation methods when working with a high number of variables compared to the number of cases. Methods A cross-sectional study conducted at the Universidade Federal de São Paulo (Federal University of São Paulo). Two Brazilian speech-language pathology and otorhinolaryngology journals (Pró-Fono and Revista Brasileira de Otorrinolaringologia) and two international speech-language pathology and otorhinolaryngology journals (Journal of Voice, Laryngoscope), all dated between 2000 and 2004, were hand-searched by specialists. Subsequently, voice and larynx publications were separated, and a speech-language pathologist and otorhinolaryngologist classified 374 articles from the four journals according to objective and study design. Results The predominant objective contained in the articles was that of primary diagnostic evaluation (27%), and the most frequent study design was case series (33.7%). A mere 7.8% of the studies were designed adequately with respect to the stated objectives. There was no statistical difference in the methodological quality of studies indexed for the ISI ® Web and the MEDLINE ® database. Conclusion The studies published in both national journals, indexed for the MEDLINE ® database, and international journals, indexed for the ISI ® Web, demonstrate weak methodology, with research poorly designed to meet the proposed objectives. There is no tool in Brazil to measure the impact of treatment on patients with OSAS. Objective To translate and culturally adapt the Sleep Apnea Quality of Life Index (SAQLI) into Brazilian Portuguese. Methods The translation and cultural adaptation were carried out in five steps: translation, synthesis of the translations, back translation, review committee and pretesting. Results A version of a culturally compatible SAQLI was constructed after lexical changes, along with changes to the sentence structures, visual format, instructions and cards. The essence of the questionnaire and its social, emotional, and disease impact in treatment measures was maintained, with 80% understanding. The final sample of 2,110 subjects from 150 different cities was enough to estimate prevalence in the Brazilian population with a sampling error of ± 2%. Questions about sleep complaints were administered face-to-face by Instituto Datafolha interviewers on March 26 and 27, 2008. Data were expanded using a weighted variable.Results:Of all interviewed subjects, 63% reported at least one sleep related complaint. Sleep complaint prevalence increased with age and was similar among inhabitants of different Brazilian regions, as well as between metropolitan areas and smaller cities. Insomnia and nightmares were significantly more prevalent in women (40% and 25%, respectively), and snoring was more prevalent in men (35%). For sleep complaints with frequencies greater than 3 times per week, we found the following prevalence: 61% for snoring, 35% for insomnia, 17% for nightmares, 53% for leg kicking, and 37% for breathing pauses.Conclusions:Because sleep disorders are affect a high proportion of the population and are known to be correlated with decreased well-being and productivity, more detailed national surveys are necessary to provide relevant information to develop approaches to prevention and treatment.Citation:Bittencourt LRA; Santos-Silva R; Taddei JA; Andersen ML; de Mello MT; Tufik S. Bronchiectasis represent a chronic disorder characterized by permanent, irreversible, abnormal dilation of the bronchi and bronchioles. Due to irreversible dilation of the bronchi, the presence of secretions and airflow obstruction, subjects with bronchiectasis may be predisposed to hypoxemia during sleep or symptoms that might lead to arousal. Therefore, we describe sleep characteristic through the standard overnight polysomnography. Methods. An observational study was carried out involving 21 patients with non-cystic fibrosis bronchiectasis at the Sleep Laboratory of the Nove de Julho University in the city of Sao Paulo, Brazil. Results. Mean age was 51.6±15.1 years; 57.1% of the patients were female and mean body mass index was 23.9±3.7 kg/m2. Mean income was 1.3 times the minimum wage and only 28.6% had completed high school. The subjects and there was a predominance of obstructive lung disease. Mean total sleep time was 282.7±69.5 min, with sleep efficiency of 79.2±29.2%. The mean sleep apnea and hypopnea index was 3.7±4.9 events/hour. The number of arousals was 5.6±2.9/h. The oxyhemoglobin desaturation index was 5.9±8.9/h and minimum oxyhemoglobin saturation was 84.5±5.8%, during sleep. Conclusion.
