the nipple and hyperthyroidism</h3> <h3 class="webcolor">Abstract</h3> <p><div bis_skin_checked="1" style="text-align: justify;">Raynaud鈥檚 phenomenon of the nipple is a rare pathology. It can occur associated with hyperthyroidism or autoimmune connective tissue diseases. We report a case associated with hyperthyroidism. A 39-year-old woman consulted for nipple pain, which worsened with breastfeeding, one month after childbirth. Raynaud鈥檚 phenomenon of the nipple was diagnosed, which improved with nifedipine. Three months later the patient developed fever. Blood test revealed thyroid stimulating hormone (TSH) 0.0008 mU/L (normal 0.55-4.75 mU/L) and free T4 48 pg/mL (normal 2.30-4.20 pg/mL). Antithyroglobulin antibodies were >500 IU/mL. T3, antiperoxidase antibodies (TPO), and thyroid-stimulating immunoglobulin were normal. Postpartum thyroiditis (PPT) was diagnosed. Two months later, TSH and free T4 levels returned to normal. Our patient presented PPT associated with Raynaud鈥檚 phenomenon. In previous studies, cases of hyperthyroidism and Raynaud's phenomenon were associated with connective tissue diseases such as diffuse or localized scleroderma, CREST syndrome, Sj枚gren's syndrome, or mixed connective tissue disease. In one case, it was associated with lymphocytic thyroiditis and in another with Graves' disease. However, we have not found studies that associate Raynaud's phenomenon with hyperthyroidism in isolation. Our patient presents with Raynaud's phenomenon of the nipple and, subsequently, postpartum thyroiditis (PPT), the latter being an autoimmune thyroid disorder. This could have been caused by different mechanisms than those previously mentioned. Unlike the cases in the literature, our patient only presents PPT as an autoimmune disease. Autoimmunity can be triggered by a genetic predisposition influenced by the environment or by an alteration of physiological mechanisms, leading to the development of autoimmune disease due to exogenous causes (molecular mimicry or antigenic hyperstimulation).<br></div></p> <h3 class="webcolor">Biography</h3> <p><div bis_skin_checked="1" style="text-align: justify;">Purificaci贸n Jordana Ferrando graduated in medicine and surgery in Barcelona, Spain, in 1988. She completed her specialty in family and community medicine in 1992.
