The present study was conducted in department of microbiology of a tertiary care hospital. Gram negative bacteria were resistant to routinely used antibiotics and were highly sensitive to Imipenem (100%). The Gram positive bacteria showed high resistance to Ampicilin (90%). The most common pathogens of bacterial sepsis and antibiotic sensitivity patterns vary in different parts of the world. The aim of this study was to determine the most common pathogens and outcome of neonatal sepsis and also antibiotic sensitivity patterns of Klebsiella species. A retrospective descriptive study was carried out. The study was performed at a neonatal care unit in Kashan between October 2000 to October 2003.Only those neonates with positive blood culture were included. Patients with Klebsiella septicemia were categorized into two groups of early and late-onset sepsis. Patterns of the antibiotic resistance of the bacterial isolates were studied by disc diffusion technique. Frequencies and Fisher&#39;s Exact test was used to compare the early-onset outcome versus late-onset outcome. One hundred and thirty-six neonates had positive blood cultures out of 453 cases. The most common pathogens were Pseudomonas, Klebsiella and coagulase negative Staphylococci respectively. Overall crude mortality rate was 39% (Pseudomonas was the predominant cause). All Klebsiella species were resistant to ampicillin. Antibiotic resistance is an increasing problem of these bacteria. The aim of this study was to detect the most common bacterial causes of neonatal sepsis and determination of their sensitivity to antibiotics. In a descriptive cross-sectional study, records of the neonates suspicious to neonatal sepsis who were admitted to Neonatal Intensive Care Unit (NICU) were assessed. Neonates with positive blood culture along with clinical signs of sepsis were entered in the study. Evaluated data included: age, sex, birth weight, type of infection, type of isolated pathogen, antibiotic sensitivity and disease outcome. Of 3700 screened blood cultures, 153 (4.1%) reported as positive, 76.5% of the evaluated neonates were preterm, 142 (92.8%) had gram negative sepsis and 11 (7.2%) had gram positive sepsis. The most common isolated gram negative bacteria were Klebsiella pneumoniae (46.4%), enterobacter spp. (17.6%) and Escherishia coli (14.4%). Coagulase negative staphylococci (CONS) were the most prevalent gram positive bacteria (5.9%) in this study. All of the klebsiella and enterobacter strains were resistant to ampicillin and gentamicin. The sensitivity of K. pneumoniae and enterobacter to imipenem was: 90 and 92.5%, respectively. Prevalence of early and late-onset sepsis was 64.7 and 35.3% respectively. Mortality rate of sepsis in this study was reported as 53.5%. In conclusion, the most common cause of bacterial sepsis in this study was K. pneumoniae which was completely resistant to ampicillin and gentamicin. The bacteriological profile of neonatal sepsis is under constant change with the advances in the diagnosis and treatment. Blood culture is still the mainstay of investigation of potential sepsis in spite of the recent advances in molecular diagnosis of fungal and bacterial sepsis. Most common bacterial organisms responsible for neonatal septicaemia in developing countries like Bangladesh, India, Nigeria, Pakistan, are Klebsiella, Escherichia coli, Enterobacter etc. The present study was undertaken to describe the spectrum of the isolates in cases of neonatal septicaemia, and their antimicrobial susceptibility pattern. The study was carried out over a one year period. Method: A total of 357 suspected patients of neonatal septicaemia were included in the study. Blood samples were collected with aseptic precautions preferably prior to the initiation of empiric antibiotic therapy. The bottles were incubated aerobically at 37 0 C for 7 days and subcultured on blood agar and Mac Conkey agar. If growth appeared the isolates were processed as per standard microbiological techniques and the isolates were identified. Antibiotic sensitivity testing was performed on Mueller‑Hinton agar plates by modified Kirby‑Bauer disk diffusion method as per Clinical Laboratory Standard Institute guidelines. Results: Of the 357 samples studied, growth was observed in 154 samples. Gram positive organisms constituted 37.66% and gram negative bacilli constituted 62.34% of the isolates. The predominant pathogens isolated were Klebsiella species, Coagulase Negative Staphylococci, Staphylococcus aureus followed by Non fermenting gram negative bacilli. 71.43% of the Staphylococcus aureus isolates were found to be methicillin resistant.96.05% of the Enterobacteriaceae were found to be Extended Spectrum Beta Lactamase (ESBL) producers. 33.33% of the Pseudomonas isolates were found to be metallo-beta-lactamase (MBL) producers. Conclusion: This study shows that Klebsiella species, Coagulase Negative Staphylococci, Staphylococcus aureus and Non-fermenting gram negative bacilli were the commonest organisms associated with Neonatal sepsis. An alarmingly high rate of MRSA and ESBL producers was observed. Gram-negative organisms were isolated in 66 cases (31.43%), and the most common Gram-negative organism causing neonatal sepsis was Klebsiella pneumoniae. The mortality rate due to Gram-negative bacteria including K. pneumoniae was higher than that due to other bacteria. It is the leading cause of neonatal mortality and morbidity in India. Aim: To isolate and identify the bacteriologic etiologic agents responsible for neonatal sepsis and to determine the susceptibility pattern of isolates at Kurnool Medical College and Govt General Hospital, Kurnool. Materials and Methods: Two hundred and fifty two samples were collected from neonates for a period of ten months and processed in accordance with standard protocols. Antibiotic susceptibility of the isolates was done by Kirby-Bauer disc diffusion method as recommended in the National Committee for Clinical Laboratory Standards (NCCLS) guidelines. Results: Blood culture reports were positive in 54.7% cases. Among the culture positive cases, Gram positive isolates were 12.6% and Gram negative isolates were 42.1%. Staphylococcus aureus was the predominant Gram positive organism isolated and Klebsiella spp. was the predominant Gram negative organism isolated. Best overall sensitivity among Gram-negative isolates was to Imipenem (95.2%) followed by Tigecycline (92.8%).Gram positive isolates had sensitivity of 96.4% to Linezolid, 82% sensitivity to Vancomycin. Conclusion: Gram negative organisms are the leading cause of neonatal sepsis in this study and most of them are multi drug resistant. For the effective management of neonatal sepsis, knowledge about causative agent and antibiotic sensitivity pattern play a vital role. Present study was undertaken to analyze the various risk factors of bacterial sepsis, to isolate aerobic bacteria causing neonatal septicemia and to study the antibiogram of bacterial isolates. Material and Methods: Study was done over a period of one year. 198 clinically suspected cases of neonatal septicemia were included in the study. Detailed record of various predisposing neonatal factors were noted in a pro-forma. Blood samples from the neonates were subjected to aerobic blood cultures. The identification of the causative organism was carried out by standard identification tests and antibiotic sensitivity testing was done as per Clinical and Laboratory Standards Institute guidelines. Results: Of 198 neonates studied, predisposing factors for neonatal sepsis were mainly low birth-weight and preterm. Blood cultures were positive in 128(64.64%) cases. Predominant organisms isolated were Staphylococcus aureus (54.7%) and Klebsiella spp. (14.1%). Among Staphylococcus aureus, 43(61.42%) isolates were MRSA and most of them were sensitive only to vancomycin and linezolid. Most of the MSSA were sensitive to cotrimoxazole, clindamycin, ciprofloxacin, vancomycin, linezolid. Most of the Gram negative bacilli were sensitive to imipenem, piperacillin-tazobactam and amikacin.
