The MO HealthNet Division has moved eligible children into one health care plan – SMHK managed by Home State Health. The SMHK health plan includes the same healthcare coverage as the MO HealthNet fee-for-service (commonly know as straight Medicaid) and other Medicaid managed care plans plus additional benefits specifically designed for the eligible population.</p><h2><span>How does it work?</span></h2><p>Individuals who qualify for coverage through SMHK are automatically enrolled in this plan and will continue to have coverage as long as they are eligible. Eligibility groups include:</p><ul><li>Children in the care and custody of the Missouri Children's Division</li><li>Youth in the custody of the Missouri Division of Youth Services</li><li>Children receiving adoption or legal guardianship subsidy</li><li>Former foster care youth under the age of 26, who were in foster care on their 18th birthday and covered by MO HealthNet (Missouri Medicaid) or Medicaid from another state, and who meet other eligibility criteria</li></ul><p><strong>Note</strong>: If you or your child is not in one of these groups or has Medicare, your child cannot be in the Show Me Healthy Kids health plan. Your child will be covered by a general MO HealthNet Managed Care health plan or the MO HealthNet Fee-For-Service program.</p><h2>What is covered?</h2><p><span>There are many benefits and services that SMHK covers. Some of these benefits are:</span></p><ul><li><span><strong>Preventive health. </strong>This includes<strong> </strong>regular medical checkups and immunizations, office visits, dental checkups and visits to the eye doctor.</span></li><li><span><strong>Behavioral health care. </strong>This includes care services for<strong> </strong>emotional, behavioral or developmental health.</span></li><li><span><strong>Hospital coverage.</strong> This means inpatient services such as hospitalizations and surgeries.</span></li><li><span><strong>Telemedicine. </strong>Providers can give you care without an in-person office visit using a computer, tablet, or smartphone.</span></li></ul><p><span>There are some services SMHK does not cover, but you will still get them through MO HealthNet (Missouri Medicaid). If the child already has healthcare through SMHK, they should have a SMHK ID card and a MO HealthNet ID card. If the child is new to SMHK (new to one of the SMHK eligibility groups) the Children’s Division will give you a Medicaid Eligibility Authorization letter that you can use to show proof of the child’s healthcare coverage until the card comes in the mail. Make sure to keep your information up to date.</p><ul><li>If you need to report a change in your contact information, or to request a new MO HealthNet ID card, contact the child’s Children’s Division case manager or subsidy case manager.</li><li>If you are a former foster care youth, email <a href="mailto:CDMHNQuestions@dss.mo.gov" class="file file-inline application-octet-stream file--application-octet-stream file--general">CDMHNQuestions@dss.mo.gov</a>.
