REUTERS/George Frey</p></div></div><div class="col col-24 col-lg-18"><div class="ArticleSummary_articleSummary__KD3Cr ArticleSummary_articleSummaryOpinion__UApU9"><p>We have the tools to beat cervical cancer but G20 leaders must commit to making sure these tools are available to all</p></div><div class="ArticleText_text__9RYj1 "><p><em>Dr Tedros Adhanom Ghebreyesus is the director-general of the World Health Organization.</em></p> <p>Every year, more than 350,000 women <a href="https://www.who.int/health-topics/cervical-cancer#tab=tab_1" target="_blank" rel="noopener">die from cervical cancer</a> and another 660,000 are diagnosed.  As a consequence, children are orphaned, families impoverished and communities diminished by the loss of mothers, wives, daughters and sisters. </p> <p>And yet, unlike most other cancers, almost all these cases and deaths can be averted.</p> <p>We have powerful vaccines that can <a href="https://www.who.int/teams/immunization-vaccines-and-biologicals/diseases/human-papillomavirus-vaccines-(HPV)" target="_blank" rel="noopener">prevent infection</a> with the human papillomavirus (HPV) that causes cervical cancer; we have diagnostics to detect it early; and we have treatments for those it strikes.</p> <p>With these tools, cervical cancer can not only be stopped; it could become the <a href="https://www.reuters.com/business/healthcare-pharmaceuticals/world-bank-gates-un-pledge-close-600m-end-cervical-cancer-2024-03-05/" target="_blank" rel="noopener">first cancer to be eliminated.</a> Some high-income countries are already close to elimination, meaning fewer than four cases per 100,000 women.</p> <p>But in many low- and middle-income countries, these tools are still not available, which is why 94% of cervical cancer deaths <a href="https://www.who.int/news-room/fact-sheets/detail/cervical-cancer" target="_blank" rel="noopener">occur in those countries.</a> The double tragedy of cervical cancer is that this is not just a health issue; it is an equity issue.</p> <p>Yesterday marked the global <a href="https://www.who.int/southeastasia/news/detail/17-11-2024-cervical-cancer-elimination-day-of-action#:~:text=November%2017%20marks%20the%20fourth,Cancer%20Elimination%20Day%20of%20Action" target="_blank" rel="noopener">“Day of Action for Cervical Cancer Elimination,”</a> with vaccination drives, screening campaigns, and advocacy events.  </p> <p>Iconic landmarks were lit up in teal, including Rio de Janeiro’s Christ the Redeemer monument, which welcomed world leaders arriving for the G20 Summit.</p> <p>In 2018, the World Health Organization <a href="https://www.who.int/initiatives/cervical-cancer-elimination-initiative" target="_blank" rel="noopener">launched a global call to action</a> to eliminate cervical cancer, which was followed in 2020 by the adoption by all 194 member states of a Global Strategy to Accelerate the Elimination of Cervical Cancer as a Public Health Problem.</p> <p>The strategy calls for countries to achieve three targets by 2030: 90% of girls fully immunized against HPV; 70% of women receiving timely screening; and 90% of those found with precancer or cancer accessing treatment.</p> <p>These targets are not just aspirational, they are achievable, even in low- and middle-income countries. Bhutan has already reached the targets, the first to do so in the South-East Asia region. Since introducing the HPV vaccine in 2011, Rwanda has <a href="https://hpvcentre.net/statistics/reports/RWA_FS.pdf" target="_blank" rel="noopener">reached vaccine coverage of 90%</a> and yesterday announced its national goal to reach the 90-70-90 targets three years ahead of schedule, by 2027. </p> <p>Nigeria, which <a href="https://www.un.org/africarenewal/magazine/october-2023/nigeria-vaccinate-77-million-girls-against-leading-cause-cervical-cancer#:~:text=In%20Nigeria%2C%20cervical%20cancer%20is,8%2C000%20deaths%20from%20cervical%20cancer" target="_blank" rel="noopener">introduced the HPV vaccine in October</a> last year, has already vaccinated 12.3 million girls.  </p> <p>We have the tools and the opportunity to eliminate cervical cancer. Realizing that opportunity requires determined political leadership. </p></div><div class="row g-0 ArticleSingleImage_wrapper__v8ERl"><div class="col-24"><div class="imageContainer ImageContainer_imageContainer__MSr4b " style="aspect-ratio:100 / 58"><img alt="U.S. President Joe Biden delivers remarks during an event at the Museum of Modern Art in Rio de Janeiro, Brazil on Monday, Nov. 18, 2024. REUTERS/Eric Lee</p></div></div><div class="ArticleText_text__9RYj1 "><h2>Political leadership</h2> <p>First, we call on G20 leaders to support access to HPV vaccines for all girls, in all countries. </p> <p>Since WHO issued the global call to action in 2018, more than 60 countries have introduced the HPV vaccine into their immunization programmes, bringing the total to 144 countries that are routinely protecting girls from cervical cancer in later life. With scientific advances, we can now prevent cervical cancer with just a single dose, which 60 countries are now doing.  </p> <p>The largest provider of HPV vaccines to low- and middle-income countries is <a href="https://www.gavi.org/vaccineswork/hpv-vaccine-lowers-risk-cervical-cancer?gad_source=1&gclid=Cj0KCQiA6Ou5BhCrARIsAPoTxrBc0tQWvbBGMSWZUyRGzZ41c8f9pDgArOPxqYI-zUilxlCHPVLMi3caAlH8EALw_wcB" target="_blank" rel="noopener">Gavi</a>, the Vaccine Alliance, which plans to vaccinate 120 million children between now and 2030. But this plan needs sustained investments in health.</p> <p>We are also counting on manufacturers to confirm and honour their commitments to provide HPV vaccines to low- and middle-income countries in the coming years.</p> <p>But we cannot rely on vaccines alone. The impact of the rapid scale-up in vaccinating girls now will not be seen for decades, when they reach the adult years when cervical cancer typically appears. To save lives now, we must also increase screening and treatment. </p> <p>So we also call on G20 leaders to support access to screening in all countries. </p> <p>Decades ago, as more women gained access to pap smears in developed countries, the mortality associated with cervical cancer dropped rapidly. Today, even better tests are available. </p> <p>Over 60 countries now include high-performance HPV tests as part of their screening programmes. Women can even collect their own samples for HPV testing, removing more barriers to life-saving services. In Australia – which is on track to become one of the first countries in the world to achieve elimination – more than a quarter of all screening tests are now done this way.</p> <p>We call on leaders to invest in organised screening programmes that will support high coverage for the whole population. This is critical for reaching the 70% target. However, high test prices and vendor mark-ups are also still a barrier.  </p> <p>Several countries are also investigating the use of artificial intelligence to enhance the accuracy of screening in resource-limited settings. When women are found with precancerous lesions, many are now treated with portable battery-powered devices, which can be operated in remote locations. </p> <p>Third, we call on G20 leaders to support expanded access to treatment in all countries. Advanced cases need to be referred for surgery, radiotherapy and palliative care. </p> <p>In many cases, women with cervical cancer die simply because the treatments are <a href="https://www.context.news/socioeconomic-inclusion/philippines-takes-cancer-screening-into-the-workplace" target="_blank" rel="noopener">not available where they live.</a> In countries where radiotherapy equipment is broken, women wait in vain as their tumors grow. Bhutan has already reached the targets, the first to do so in the South-East Asia region. Since introducing the HPV vaccine in 2011, Rwanda has\u0026nbsp;\u003ca href=\"https://hpvcentre.net/statistics/reports/RWA_FS.pdf\" target=\"_blank\" rel=\"noopener\"\u003ereached\u0026nbsp;vaccine coverage of\u0026nbsp;90%\u003c/a\u003e and yesterday announced\u0026nbsp;its national\u0026nbsp;goal\u0026nbsp;to\u0026nbsp;reach\u0026nbsp;the 90-70-90\u0026nbsp;targets\u0026nbsp;three years ahead of schedule,\u0026nbsp;by 2027.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003eNigeria, which \u003ca href=\"https://www.un.org/africarenewal/magazine/october-2023/nigeria-vaccinate-77-million-girls-against-leading-cause-cervical-cancer#:~:text=In%20Nigeria%2C%20cervical%20cancer%20is,8%2C000%20deaths%20from%20cervical%20cancer\" target=\"_blank\" rel=\"noopener\"\u003eintroduced the HPV vaccine\u0026nbsp;in October\u003c/a\u003e last year,\u0026nbsp;has already\u0026nbsp;vaccinated\u0026nbsp;12.3 million girls.\u0026nbsp;\u0026nbsp;\u003c/p\u003e\n\u003cp\u003eWe\u0026nbsp;have\u0026nbsp;the tools and the\u0026nbsp;opportunity\u0026nbsp;to\u0026nbsp;eliminate\u0026nbsp;cervical cancer.\u0026nbsp;Realizing that opportunity\u0026nbsp;requires\u0026nbsp;determined\u0026nbsp;political leadership.\u0026nbsp;\u003c/p\u003e","bodyPart21":"\u003ch2\u003ePolitical leadership\u003c/h2\u003e\n\u003cp\u003eFirst,\u0026nbsp;we call on\u0026nbsp;G20 leaders to support\u0026nbsp;access to HPV\u0026nbsp;vaccines for\u0026nbsp;all girls, in all countries.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003eSince WHO issued the global\u0026nbsp;call to action\u0026nbsp;in 2018, more than 60 countries have introduced the HPV vaccine into their immunization programmes, bringing the total to 144 countries that are routinely protecting girls from cervical cancer in later life.\u0026nbsp;With scientific advances, we can now prevent cervical cancer with just a single dose, which 60 countries\u0026nbsp;are now doing. \u0026nbsp;\u003c/p\u003e\n\u003cp\u003eThe largest provider of HPV vaccines to low- and middle-income countries is\u0026nbsp;\u003ca href=\"https://www.gavi.org/vaccineswork/hpv-vaccine-lowers-risk-cervical-cancer?gad_source=1\u0026amp;gclid=Cj0KCQiA6Ou5BhCrARIsAPoTxrBc0tQWvbBGMSWZUyRGzZ41c8f9pDgArOPxqYI-zUilxlCHPVLMi3caAlH8EALw_wcB\" target=\"_blank\" rel=\"noopener\"\u003eGavi\u003c/a\u003e,\u0026nbsp;the Vaccine Alliance,\u0026nbsp;which\u0026nbsp;plans to vaccinate 120 million\u0026nbsp;children\u0026nbsp;between now and\u0026nbsp;2030.\u0026nbsp;But this plan\u0026nbsp;needs sustained investments in health.\u003c/p\u003e\n\u003cp\u003eWe are also counting on manufacturers to confirm and\u0026nbsp;honour\u0026nbsp;their commitments\u0026nbsp;to\u0026nbsp;provide\u0026nbsp;HPV vaccines to\u0026nbsp;low- and middle-income countries\u0026nbsp;in the coming years.\u003c/p\u003e\n\u003cp\u003eBut we cannot rely on vaccines alone.\u0026nbsp;The impact of the rapid scale-up in vaccinating girls now will not be seen for decades, when they reach the adult years when cervical cancer typically appears.\u0026nbsp;To save lives now,\u0026nbsp;we must\u0026nbsp;also increase\u0026nbsp;screening and treatment.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003eSo\u0026nbsp;we also call on\u0026nbsp;G20\u0026nbsp;leaders\u0026nbsp;to support\u0026nbsp;access to\u0026nbsp;screening in\u0026nbsp;all countries.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003eDecades ago, as more women\u0026nbsp;gained\u0026nbsp;access to pap smears\u0026nbsp;in developed countries, the mortality associated with cervical cancer dropped\u0026nbsp;rapidly.\u0026nbsp;Today, even better tests are available.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003eOver 60 countries now include\u0026nbsp;high-performance\u0026nbsp;HPV tests\u0026nbsp;as part of their screening programmes.\u0026nbsp;Women\u0026nbsp;can\u0026nbsp;even\u0026nbsp;collect their own samples for HPV testing, removing\u0026nbsp;more barriers to life-saving services.\u0026nbsp;In Australia\u0026nbsp;\u0026ndash;\u0026nbsp;which\u0026nbsp;is on track to become one of the first\u0026nbsp;countries\u0026nbsp;in the world to\u0026nbsp;achieve elimination\u0026nbsp;\u0026ndash;\u0026nbsp;more than a quarter of all screening tests are now done this way.\u003c/p\u003e\n\u003cp\u003eWe call on leaders to invest in organised screening programmes that will support high coverage for the whole population. This is critical for reaching the 70% target.\u0026nbsp;However, high test prices and vendor mark-ups are\u0026nbsp;also\u0026nbsp;still a barrier. \u0026nbsp;\u003c/p\u003e\n\u003cp\u003eSeveral countries are\u0026nbsp;also\u0026nbsp;investigating the use of\u0026nbsp;artificial intelligence\u0026nbsp;to enhance the\u0026nbsp;accuracy\u0026nbsp;of\u0026nbsp;screening\u0026nbsp;in resource-limited settings.\u0026nbsp;When women are found with precancerous lesions, many are now treated with portable battery-powered devices, which can be operated in remote locations.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003eThird, we call on\u0026nbsp;G20\u0026nbsp;leaders to\u0026nbsp;support\u0026nbsp;expanded\u0026nbsp;access to treatment\u0026nbsp;in all countries.\u0026nbsp;Advanced cases need to be referred for surgery,\u0026nbsp;radiotherapy\u0026nbsp;and palliative care.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003eIn many cases, women with cervical cancer die simply because the treatments are \u003ca href=\"https://www.context.news/socioeconomic-inclusion/philippines-takes-cancer-screening-into-the-workplace\" target=\"_blank\" rel=\"noopener\"\u003enot available where they live.\u003c/a\u003e\u0026nbsp;In countries where radiotherapy equipment is broken, women wait in vain as their tumors grow.
