The prevalence of asthma in the world is increasing. Although there are good controller drugs major concern about the side effects still remains. The asthma prevalence and clinical features showed gender related differences. It is not clearly known whether the difference is due to sex steroids or the way the drug is used or even multifactorial causes. But what about responses to treatment? Are same treatments have same responses in two gender? To answer this question, we obtained this study to evaluate the effect of gender in new herbal formulation in mild asthma patients. we selected one of the ancient prescriptions from Persian Medicine. The participants divided to three groups randomly. One group received fenugreek syrup one received honey syrup and the third received sugar syrup as placebo. Duration of treatment was four weeks. 90 patients with mild asthma entered the study but only 79 completed the study duration. This number included 49 women and 30 men. Asthma control test, St. Gorge questioner, spirometry parameters and IL4 were checked before and after the study. At the beginning of the study women had lower scores in St. Gorge and ACT results but at the end of the study St. Gorge scores improved in men more than women but it was not statistically significant. There weren’t significant differences between men and women in IL4 and spirometry results. although the significant response in fenugreek and honey syrup groups exist, sex didn’t have significant
