I understand that I am prohibited from recording electronic interaction with my clinician.</p> <p>I understand that attachments or image captures may be used to assist in communications with a clinician and should not be used as a substitute for in person consultation with a clinician or in lieu of seeking emergency services.</p> <p><strong>For all urgent matters that I believe may immediately affect my health or well-being, I will, without delay, contact my clinician, and/or go to the emergency department of a local hospital, and/or dial 911.</strong></p> <p>I understand that my healthcare team may send me secure e-Messages via my account. These messages may contain information that is important to my health and medical care. It is my responsibility to monitor these messages. By entering my valid and functional e-mail address, I have enabled Sutter to notify me of messages sent to my Inbox. I understand that maintaining my current contact information with Sutter's online patient services is my responsibility and I will update my e-mail address on my account as needed.</p> <p><strong>Contents of Electronic Messages</strong></p> <p>It is always best for you and your clinician to agree on the type of electronic communication that is most appropriate in your particular case. Using electronic communication for solicitation purposes is prohibited. All Sutter-generated EHR content is subject to state and federal statutes governing the security and confidentiality of health records.</p> <p><strong>Electronic Interactions</strong></p> <p>Messages and attachments you send via your online account or images captured via video interaction with your clinician may be incorporated in your permanent health record. Once a message, attachment or image capture is made part of your record, it will be accessible to current and future Sutter staff members who are involved in your care. If your Sutter clinician is out of the office or unavailable to respond, messages sent may be routed to other authorized clinicians within Sutter in order to facilitate a timely response to your request or question.
