left"/></a><p>There is no doubt that the United States has shown tremendous leadership and brought about remarkable results in the global fight against AIDS over the past decade. U.S. investments through the Presidents Emergency Plan for AIDS Relief (<a href="https://web.archive.org/web/20120314124029/http://www.pepfar.gov/">PEPFAR</a>) have strengthened overall health systems, built and sustained capacity to address HIV and a host of other diseases, and helped scale up treatment to save millions of lives. While innovative prevention efforts are still needed to achieve an <a href="https://web.archive.org/web/20120314124029/http://www.cgdev.org/content/publications/detail/1424143">AIDS transition</a> (e.g. a sustained decline in the total number of people living with HIV/AIDS), it seems we have reached a “tipping point” where the science, technology and know-how are available to realistically talk about creating an <a href="https://web.archive.org/web/20120314124029/http://www.state.gov/secretary/rm/2011/11/176810.htm">AIDS free generation</a>, as Secretary of State Clinton did a few weeks ago. In keeping with CGD standards, I can’t sign off with just a plain old goodbye and thank you post, or dwell on the gloom and doom about funding for global health (see <a href="https://web.archive.org/web/20120314124029/http://www.google.com/hostednews/afp/article/ALeqM5ilAj_3kL_DMW5iWg4VkFGSLjnqfw?docId=CNG.8af6b7bf5835350c0f276c56855993c1.71">here</a>) and development ODA in general (see <a href="https://web.archive.org/web/20120314124029/http://www.guardian.co.uk/global-development/datablog/2011/nov/07/us-foreign-aid-budget-cuts">here</a> and <a href="https://web.archive.org/web/20120314124029/http://www.ft.com/cms/s/0/3749cf54-18dd-11e1-92d8-00144feabdc0.html#axzz1f1qxd86Y">here</a>). Why? </p> <p>Four very good reasons:</p> <p>1) At scale intervention, 2) Easy to administer, 3) Low-cost, and 4) multi-impact</p> <p>It’s always exciting to read about proven development solutions that go to scale because of their potential impact&#8211;in this case, a state-wide (Bihar) school-based de-worming program targeting all 21 million school-age children. To put that number into perspective, it’s almost the size of Australia’s population! More importantly, we rarely hear about large-scale programs that involve proven interventions that have spillover benefits. Bihar’s state government implemented a simple (1 or 2 tablets per child per year), safe, low-cost (~ $0.50 per child per year), and <a href="https://web.archive.org/web/20120314124029/http://weber.ucsd.edu/~tkousser/Miguel%20and%20Kremer.pdf" title="proven">proven</a> effective intervention to keep worms out of little children, and to keep children in school. This was the topic: “Countries should spend a majority of what is likely to be a flat or even declining HIV prevention budget on ‘treatment as prevention.’” The pro and con sides were each represented by two eminent and articulate medical doctor/scientist/researcher/public health experts. Whether this child is <a href="https://web.archive.org/web/20120314124029/https://www.facebook.com/meetmaya">Maya</a> (the World Bank’s virtual seven billionth child) or <a href="https://web.archive.org/web/20120314124029/http://www.guardian.co.uk/world/2011/oct/31/seven-billionth-baby-born-philippines">Danica</a> (the UN’s symbolic but ‘real life’ seven billionth child from the Philippines), the arrival of the world’s seven billionth person has led to a renewed interest in population across many sectors of government, health, and development work.</p> <p>Reaching such a substantial milestone was bound to bring about mixed reactions—and it did, from the “more the merrier” <a href="https://web.archive.org/web/20120314124029/http://www.foreignpolicy.com/articles/2011/05/23/more_people_please">musings</a> of Charles Kenny to <a href="https://web.archive.org/web/20120314124029/http://www.nytimes.com/2011/10/24/opinion/seven-billion.html?scp=2&amp;sq=seven%20billion&amp;st=cse">concerns</a> regarding food security and increasing poverty. Launched in 2008, India’s health insurance program for the poor, <em>Rashtriya Swasthya Bima Yojna</em> (<a href="https://web.archive.org/web/20120314124029/http://www.rsby.gov.in/">RSBY</a>) is the subject of a <a href="https://web.archive.org/web/20120314124029/http://www.cprindia.org/publications/books/3348-indias-health-insurance-scheme-poor-evidence-early-experience-rashtriya-swas">new book</a> edited by Robert Palacios, Jishnu Das and Changqing Sun. According to the authors, RSBY covers 25 million families (75 million people) below the poverty line and has subsidized 2.7 million hospital visits. An overall (if varied) increase in demand for hospital services among RSBY families has prompted a supply increase in some underserved districts. The handful of surveys conducted so far have generally indicated good patient satisfaction.
