In 2022, the <a href="https://www.who.int/news/item/06-08-2013-who-releases-guidance-on-mental-health-care-after-trauma#:~:text=Traumatic%20events%20and%20loss%20a%20common%20experience&amp;text=An%20estimated%203.6%25%20of%20the,previous%20year%2C%20the%20study%20showed."><strong>World Health Organization</strong></a> (WHO) estimates approximately 3.6 percent of 250 million people of the global population experienced PTSD and the associated symptoms of anxiety and depression. </p> <p>Still, nothing creates change better than a crisis. Throughout the pandemic and into today, mental health awareness has come out of the shadows, becoming a spotlight issue that millions of people openly discuss and advocate fiercely for equal access to treatment. Furthermore, the traditional Western medicine model for treating these symptoms is often ineffective as it is not designed to treat the root cause, which is in the mind.&nbsp;</p> <p><a href="https://thecitesite.com/authors/eckhart-tolle/"><strong>Eckhardt Tolle</strong></a> said, “One of the main tasks of the mind is to fight or remove the emotional pain, which is one of the reasons for its incessant activity, but all it can ever achieve is to cover it up temporarily. In an effort to explain how acupressure works on the body, it is important to understand three fundamental concepts within TCM: Yin and Yang, Qi,  and meridians.</p> <h4 class="wp-block-heading"><strong><em>Yin and Yang</em></strong></h4> <p><a href="https://www.nlm.nih.gov/exhibition/chinesemedicine/yin_yang.html#:~:text=Yin%20and%20Yang%20are%20the,with%20particular%20reference%20to%20acupuncture."><strong>Yin and Yang</strong></a> represent the dual nature of everything in the universe. Yin is associated with darkness, stillness, and cold, while Yang represents light, activity, and heat. Health is achieved when there is a balance between Yin and Yang within the body. An imbalance can lead to illness, and TCM treatments, including acupressure, aim to restore this balance.</p> <h4 class="wp-block-heading"><strong><em>Qi (Vital Life Force)</em></strong></h4> <p><a href="https://www.amcollege.edu/blog/qi-in-traditional-chinese-medicine"><strong>Qi (pronounced <em>“Chi”</em>)</strong></a> is the vital life force or energy that flows through the body along pathways called meridians. It is essential for maintaining physical, mental, and emotional well-being. When Qi is blocked or imbalanced, it can result in various health problems. TCM treatments focus on balancing and harmonizing the flow of Qi to promote health and healing.</p> <h4 class="wp-block-heading"><strong><em>Meridians</em></strong></h4> <p><a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6448339/"><strong>Meridians</strong></a> are energy pathways that run throughout the body, connecting various organs and tissues. There are 12 main meridians, each associated with specific organs. Along these meridians are <a href="https://www.medicalnewstoday.com/articles/what-is-acupressure"><strong>acupoints</strong></a>, which are specific points where Qi can be accessed and manipulated. Stimulating these points through techniques like acupressure helps restore the flow of Qi and balance Yin and Yang.</p> <h4 class="wp-block-heading"><strong><em>Acupressure and TCM</em></strong></h4> <p><a href="https://sageintegrativewellness.com/the-benefits-of-acupressure-for-stress-reduction-during-the-holidays/"><strong>Acupressure</strong></a> is a technique that involves applying pressure to specific acupoints on the body. By pressing these points, the flow of Qi within the meridians can be regulated, promoting balance between Yin and Yang. Additionally, it is worth noting that the difference between <a href="https://pulmonaryfibrosisnow.org/pfnowtest/acupuncture-vs-acupressure-whats-the-difference/#:~:text=Acupuncturists%20use%20hair%2Dthin%20needles,stimulate%20different%20meridians%2C%20or%20acupoints."><em><strong>acupuncture</strong></em></a> and acupressure is that acupuncture uses hair-thin needles to stimulate a specific acupoint. In contrast, acupressure uses NO needles and is performed using hand and gentle touch. More importantly, both acupressure and <em>acupuncture</em> stimulate acupoints to promote healing. </p> <h4 class="wp-block-heading"><strong><em>Acupressure and Western Medicine</em></strong></h4> <p>In Western medicine, there is a debate on exactly how acupressure works on the body. According to John Hopkins Medicine, acupoints <a href="https://www.hopkinsmedicine.org/health/wellness-and-prevention/acupuncture"><strong>stimulate the central nervous system</strong></a> to release chemicals into the muscles, spinal cord, and brain.  While another study found that acupressure meridians utilize the <a href="https://www.hopkinsmedicine.org/health/wellness-and-prevention/muscle-pain-it-may-actually-be-your-fascia#:~:text=Fascia%20is%20a%20thin%20casing,almost%20as%20sensitive%20as%20skin."><strong>extracellular matrix and fascia</strong></a> (thin-connective tissue covering organs, blood vessels, bones, nerve, and muscle fibers) to exert their effects. However, one idea Western medicine can agree on is the effects of acupressure therapy. </p> <h3 class="wp-block-heading"><strong>Acupressure for PTSD, Anxiety, and Depression: Evidence</strong></h3> <p>First, let us explore the scientific evidence for using acupressure for PTSD, anxiety, and depression. In June 2020, the scientific journal Medicine published a research article, <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7328930/#:~:text=According%20to%20the%20research%2C%20acupuncture,relieve%20the%20symptoms%20of%20PTSD."><strong>Efficacy and safety of acupuncture in treating post-traumatic stress disorder</strong>,</a> which concluded that acupuncture can affect the autonomic nervous system (the nervous system that controls our breathing, heart rate, and blood pressure) and the prefrontal (judgment and decision-making) as well as limbic (fear-inducing) brain structures, thus reducing the symptoms of PTSD. Additionally, fatigue and insomnia, often accompanied by depressive symptoms, were significantly reduced by stimulation of several acupoints for 12 minutes three times a week for four weeks. In further evidence, <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6494180/"><strong>Acupuncture for depression</strong></a> found acupuncture may result in a moderate reduction in the severity of depression compared to treatment as usual or no treatment at all. Moreover, research shows that pressure applied at ST-36 evokes a <a href="https://www.researchgate.net/publication/333033416_Acupuncture_in_the_Treatment_of_Post-Traumatic_Stress_Disorder_A_Neurobiological_Approach"><strong>deactivation in the amygdala and hippocampus</strong></a>, reducing the body’s expression of fear and irritability and easing the mind’s grip on traumatic memories. </p> <p>Next, the following pictures are examples of three acupoints used frequently in caring for PTSD, anxiety, and depression. If you choose to stimulate these acupoints, please find an intentional time to hold points, connect with a point using a gentle touch, consider holding for a few breaths, and release to the next point as needed. At the end of your self-care, journal anything you need to process what you discover while working with acupoints on your body.</p> <p><strong>GB-21 is called “Jian Jing,” or shoulder wall</strong> stimulation of this acupoint has been shown to reduce the sensation of pain and anxiety. GB21 is beneficial for pain/tension reduction/releasing things physically and emotionally that keeps us tense. In fact, among adults with chronic pain in states with medical cannabis laws, 3 out of 10 people reported<strong> <a href="https://jamanetwork.com/journals/jamanetworkopen/fullarticle/2800119">using cannabis to manage their pain</a>. </strong></p> <p>Understanding the types of pain, the role of the endocannabinoid system (ECS) in pain modulation, and how to use medical cannabis can help you achieve holistic pain relief.&nbsp;</p> <h3 class="wp-block-heading">Types of Pain</h3> <p>Throughout our bodies are specialized sensory receptors called nociceptors which alert our brain to painful or potentially harmful stimuli. When stimulated they produce different types of pain. The following are some examples of pain type:</p> <p><a href="https://www.iasp-pain.org/resources/topics/acute-pain/#:~:text=What%20is%20Acute%20Pain%3F,to%20less%20than%20six%20months."><strong>Acute pain</strong></a> is short-term pain that usually lasts for less than three months. It is caused by an injury, surgery, or illness and is often described as sharp, intense, and sudden. Most importantly, medical cannabis is not recommended for treating acute pain.&nbsp;</p> <p><a href="https://www.hopkinsmedicine.org/health/conditions-and-diseases/chronic-pain"><strong>Chronic pain</strong></a> persists for more than three months. Various conditions, such as arthritis, fibromyalgia, or nerve damage, cause it.&nbsp;</p> <p><a href="https://www.cancer.org/treatment/treatments-and-side-effects/physical-side-effects/pain/other-types.html"><strong>Breakthrough pain</strong></a> is a flare of pain that happens even though you regularly take pain medicine for acute or chronic pain.</p> <p><a href="https://my.clevelandclinic.org/health/diseases/15833-neuropathic-pain"><strong>Neuropathic pain</strong></a> is caused by damage or injury to the nerves. It can result in burning, stabbing, or shooting pain. Neuropathic pain can be caused by conditions such as diabetes, multiple sclerosis, or nerve damage due to chemotherapy or radiation therapy.&nbsp;</p> <p><a href="https://www.ninds.nih.gov/health-information/disorders/migraine"><strong>Migraines</strong></a> are headaches characterized by recurrent attacks of moderate to severe throbbing and pulsating pain on one side of the head. The pain is caused by the activation of nerve fibers within the wall of brain blood vessels traveling inside the meninges (three layers of membranes protecting the brain and spinal cord).</p> <p><a href="https://www.sciencedirect.com/topics/medicine-and-dentistry/inflammatory-pain#:~:text=Inflammatory%20pain%20is%20the%20spontaneous,pain%2C%20trauma%2C%20arthritis)."><strong>Inflammatory pain</strong></a> is caused by inflammation in the body. It can result from conditions such as arthritis and autoimmune diseases like Crohn&#8217;s disease or lupus.</p> <p><a href="https://www.medicalnewstoday.com/articles/321835"><strong>Bone pain</strong></a> is aching tenderness, or another discomfort in the bone. One of the most common symptoms of bone cancer is bone pain.</p> <h3 class="wp-block-heading">The role of the ECS in pain</h3> <p>The <a href="https://www.phytecs.com/wp-content/uploads/2015/02/Russo-Introduction-to-the-Endocannabinoid-System-corr-January-2015.pdf"><strong>endocannabinoid system</strong></a> (ECS) is a complex network of cannabinoid receptors, endocannabinoid molecules, and enzymes that play a crucial role in pain processing and regulation. In fact, the ECS exhibits <a href="https://pubmed.ncbi.nlm.nih.gov/25846617/#:~:text=The%20endocannabinoid%20(EC)%20system%20is,stages%20of%20pain%20processing%20pathways."><strong>anti-nociception</strong></a> abilities that reduce pain sensations with regulatory actions at all stages of pain processing pathways. </p> <p>There are two main types of cannabinoid receptors known as CB1 and CB2. CB1 receptors heavily populate the pain-processing area of the brain and central nervous system which control our perception of pain. While CB2 receptors heavily populate our immune system and the peripheral nervous system and work to reduce inflammatory pain. Additionally, these CB receptors act in concert with other pain-modulating receptors, such as our body’s opioid receptors&nbsp; (more on this later in the article) to alter the pain experience. Finally, Transient Receptor Potential Cation Channel Subfamily V member 1 (TRPV1) plays a part in mediating inflammation, pain perception, and body temperature. Additionally, both act exhibit anti-inflammatory properties by preventing the production of chemicals that cause <a href="https://sageintegrativewellness.com/cannabis-for-pain-and-inflammation-confessions-of-a-cbd-nurse-educator/"><strong>chronic inflammation</strong></a>. Finally, cannabinoids act on nerve and non-nerve tissues to help reduce the sensations caused by neuropathic pain. </p> <h3 class="wp-block-heading">Cannabis and reducing opioid use&nbsp;</h3> <p>Opioid crisis and the lack of effective treatment pushed cannabis to the forefront as a possible exit-drug for those struggling with addiction. In 2016, a study released by the University of Michigan revealed a <a href="https://www.ncbi.nlm.nih.gov/books/NBK574562/"><strong>statistic of reduction of opiate use</strong></a> by 64% amongst chronic pain patients who used medical marijuana. Then in 2021, a collaborative study from University of California, Davis and Yale University found an increase from one to two storefront dispensaries in a county is associated with an estimated 17% reduction in all opioid related mortality rates. Given these statistics, it is easy to see that cannabis is a supportive therapy to reducing and even eliminating the chronic use of opioids.   </p> <h3 class="wp-block-heading">How to use cannabis for holistic pain management</h3> <p>The five most common ways to use cannabis for holistic pain management are:</p> <ul> <li><strong>Inhalation</strong> or smoking cannabis is the fastest way to experience pain relief. However, it is also the fastest way cannabis is eliminated in the body so those who smoke find that after 2 hours their pain may increase again. Cannabis communities like <a href="https://www.leafly.com/"><strong>Leafly</strong></a> offer reviews of different chemovar (strains) from thousands of people also seeking pain relief.</li> </ul> <ul> <li><strong>Ingestion </strong>or taking cannabis by mouth is a discrete method that can provide 4 to 6 hours of pain relief. However, onset can take up to 90 minutes before the body may experience relief. Additionally, ingestion of THC requires smaller doses because THC is converted to a more potent metabolite once it reaches the liver. It is this active, potent metabolite that affords the hours of relief. Common products are CBD oil,&nbsp; THC oil, gummies, chocolate, and even drinks.&nbsp;&nbsp;</li> </ul> <ul> <li><strong>Transdermal Patches </strong>are applied to skin and are a novel way to achieve a consistent dose over several hours and even days. <a href="https://www.simonandschuster.com/books/Healing-with-CBD/Eileen-Konieczny/9781612438290"><strong>Transdermal effects can be felt as soon as 30 minutes to an hour, and last as long as 3 days.</strong></a> Transdermal patches come in a variety of doses of THC, CBD, or a combination of both, so it is easy to find the right one to fit your pain relief needs. Additional transdermal products include gels and lotions, however the effects of these products usually only last 2 to 4 hours.</li> </ul> <ul> <li><strong>Suppositories</strong> work by allowing cannabinoids to pass through the mucosal layer in the rectum or vagina. Additionally, they are a great cannabis delivery option for people who can no longer swallow. Suppositories begin to work within 30 minutes to 1 hour and the effects can last between 5 to 8 hours.&nbsp;</li> </ul> <ul> <li><strong>Topical</strong> application is applied to skin and is great for joint pain, minor burns, and rashes. Topically applied cannabis begins to take effect between 15 to 30 minutes and lasts between 2 and 4 hours.&nbsp;</li> </ul> <p>One important aspect of understanding the onset (when it begins to work) and the duration (how long it lasts) of ways to use cannabis as medicine is the ability to layer doses. For example, your pain level is at an 8 on a scale from 0 to 10 with ten being the worst. You can smoke a couple puffs on a pipe and ingest a 5 mg gummy. By the time the effects of the smoked cannabis starts to wear off, the ingested effects will begin thus, giving you immediate yet, longer lasting relief.&nbsp;&nbsp;</p> <h3 class="wp-block-heading">How to start dosing cannabis</h3> <p>Understanding how to choose the right cannabis dose is vital to optimal pain relief. As a holistic cannabis nurse, the recommendation is to “start low and go slow.” If you are new to cannabis, start with the lowest dose, such as 1-2 mg, and slowly increase the dose amount until you achieve the effect you desire.&nbsp;</p> <p>Cannabis works in a “biphasic” manner. This scientific word simply means that too small of dose may cause unwanted side effects or ineffective pain relief. Likewise, too large of a dose may cause unwanted side effects or an INCREASE in pain. Next, <strong><a href="https://my.clevelandclinic.org/health/articles/22572-serotonin#:~:text=Serotonin%20is%20a%20chemical%20that,blood%20clotting%20and%20sexual%20desire.">Serotonin</a> </strong>is the neurotransmitter that plays a role in mood, sleep, digestion, nausea, wound healing, bone health, blood clotting, and sexual desire. Lastly, <strong><a href="https://my.clevelandclinic.org/health/articles/22857-gamma-aminobutyric-acid-gaba">GABA</a> </strong>(Gamma-aminobutyric acid) is a neurotransmitter that slows down your brain by blocking specific signals in your central nervous system (the brain and spinal cord). Notably, <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7696928/#:~:text=Anthocyanins%20exhibit%20a%20significant%20neuroprotective,both%20homeostatic%20and%20pathological%20circumstances."><strong>anthocyanins</strong></a>, a flavonoid found in red, blue, and purple edible plants, exhibit neuroprotective properties that protect against our normal aging process, may prevent dementia, and improve our memory. Another study found a <a href="https://content.iospress.com/articles/journal-of-berry-research/jbr220054"><strong>diet high in anthocyanins</strong></a> supports good mental health through improved sleep, lower perceived stress, and decreased depressive symptoms.</p> <h3 class="wp-block-heading"><strong>Wild-Caught Salmon</strong></h3> <p><a href="https://scienceline.org/2014/03/why-is-salmon-good-for-your-brain/"><strong>Wild-Caught Salmon</strong></a> is a rich source of omega-3s which supports optimal cognition, such as memory and learning. Realistically, more than two-thirds of the brain&#8217;s fatty acids are docosahexaenoic acid (DHA), an omega-3 fatty acid found primarily in oily fish like wild-caught salmon. However, some vegetarian sources exist as well such as seaweed, walnuts, hemp and chia seeds, and kidney beans. Yet, to generalize all traumatic brain injury survivors into one category would be an injustice. Classifying TBIs is a complex, multi-faceted process. <a href="https://bestpractice.bmj.com/topics/en-us/515"><strong>The British Journal of Medicine’s Best Practice</strong></a> instructs healthcare providers to evaluate TBIs by type, severity, location, cause, and the body’s physical response to injury. </p> <p>The complexity of evaluation equates to each traumatic brain injury being as unique as each survivor.&nbsp;</p> <h3><strong>Types and Causes of TBI </strong></h3> <p>Physicians classify TBIs into two broad types: <a href="https://www.biausa.org/brain-injury/about-brain-injury/basics/overview"><strong>traumatic and non-traumatic</strong></a>. </p> <p>Traumatic Brain injuries are caused by blunt force head trauma, penetrating objects, and blast injuries. <a href="https://my.clevelandclinic.org/health/diseases/8874-traumatic-brain-injury"><strong>Falls and car accidents</strong></a> are the two top causes of traumatic brain injuries (TBIs). However, the most common type of brain injury is a concussion. </p> <p><a href="https://www.biausa.org/brain-injury/about-brain-injury/basics/overview"><strong>Non-traumatic injuries</strong></a> are caused by internal forces that cause damage to the brain, such as a lack of oxygen or increased pressure on the brain. Common causes of non-traumatic brain injuries are strokes, seizures, infections, and near-fatal drownings. However, some closed-head injuries are more insidious, like tumors and aneurysms, which slowly develop over time. </p> <h3><strong>Traditional Medicine Treatments</strong></h3> <p>Traditional traumatic brain injury treatments include diagnostic imaging, pharmaceuticals, and specialized brain rehabilitation therapies.&nbsp;</p> <h4>Diagnostic Imaging </h4> <p>Diagnostic imaging is crucial in determining whether a patient has a mild traumatic brain injury or a more severe brain injury, especially when there are no visual signs of head trauma.&nbsp;</p> <p><a href="https://www.mayoclinic.org/diseases-conditions/traumatic-brain-injury/diagnosis-treatment/drc-20378561"><strong>Computed tomography (CT)</strong></a> scan is the most commonly used imaging modality for TBI. CT imaging uses X-rays to create detailed images of the brain. CT scans can detect fractures, bleeding, and swelling, which can help guide traumatic brain injury treatment decisions.</p> <p>Next, <a href="https://www.mayoclinic.org/diseases-conditions/traumatic-brain-injury/diagnosis-treatment/drc-20378561"><strong>Magnetic resonance imaging (MRI)</strong></a> is another imaging modality used to evaluate TBI. Unlike CT scans, MRI uses a magnetic field and radio waves to create detailed brain images. MRI is particularly useful in detecting subtle changes in brain tissue, such as a bruised brain or damage to the white matter.</p> <p>In addition to CT scans and MRIs, <a href="https://www.amenclinics.com/approach/why-spect/"><strong>Single Photon Emission Computed Tomography (SPECT)</strong></a> is a highly-specialized imaging test that uses a small amount of radioactive material to diagnose and manage traumatic brain injury. According to <a href="https://www.amenclinics.com/approach/why-spect/"><strong>Dr. Daniel Amen, SPECT</strong></a> scans allow physicians to look deep inside the brain to observe three things: (1) Areas of the brain that work well, (2) Areas of the brain that work too hard, and (3) Areas of the brain that do not work hard enough. </p> <p>Finally, diagnostic imaging does not provide a diagnosis, instead, the imaging aids physicians in creating an effective plan of care, that includes pharmaceutics and specialized brain rehabilitation therapies, that would most&nbsp; benefit the traumatic brain injury survivor.&nbsp;</p> <h4>Pharmaceuticals </h4> <p>While many medications are available to TBI survivors, we will focus on the most commonly prescribed pharmaceuticals: anti-seizure, anti-depressants, and anti-anxiety drugs.&nbsp;</p> <p>Seizures, whether from Epilepsy or as a result of a traumatic brain injury, must be controlled to prevent further brain injury. Anti-seizure medications aim to help control seizure activity without unwanted side effects. Physicians prescribe anti-depressants or anti-anxiety medications that work to balance chemicals in the brain called neurotransmitters.  Furthermore, it is crucial to incorporate mental health counseling while using anti-depressants or anti-anxiety medications to develop healthy coping skills specific to your neurodiversity.</p> <h4>Specialized Brain Rehabilitation Therapies </h4> <p>Most TBI survivors receive one or more specialized brain rehabilitation therapies (SBRT), such as physical therapy, occupational therapy, and speech therapy, to regain or improve skill sets affected by their traumatic brain injury.&nbsp;</p> <p>The <a href="https://www.biausa.org/"><strong>Brain Injury Association of America</strong></a> (BIAUSA) estimates nearly half of all TBI patients experience <a href="https://www.biausa.org/public-affairs/media/the-vestibular-system-finding-your-balance"><strong>vestibular impairments,</strong></a> like dizziness, vertigo, and feeling off-balanced. Still, there is hope physical therapists trained in vestibular therapy evaluate the deficits created by traumatic brain injury and craft an individual treatment plan that helps body movements become fluid and balanced. </p> <p>Another SBRT utilized by many TBI survivors is occupational therapy (OT). OT focuses on cognitive rehabilitation, which helps patients live more independently and accomplish goals such as the ability to work, drive a car, cook for themselves, manage money, and participate in recreational activities. Also, SLPs may evaluate a patient’s ability to swallow and eat.
