href="https://web.archive.org/web/20090925035949/http://blogs.cgdev.org/globalhealth/2009/07/wedding-bells-for-gavi-the-world-bank-and-the-global-fund.php">yesterday’s post</a> about the Global Fund, GAVI and the World Bank (all from individuals who didn’t want to post a comment publicly), I got one question: “Why did you say ‘<em>self-proclaimed</em> comparative advantage in financing and systems issues’? The Bank obviously has the comparative advantage on those topics.”</p> <p>How obvious the Bank’s comparative advantage is depends on the answer to the question “compared to what”?</p> <p>Compared to the Global Fund to Fight AIDS, TB and Malaria and the GAVI Alliance, two organizations established to collect and distribute money for disease-specific programs in developing countries, it’s undoubtedly true that the Bank has a set of assets that make it better suited to engaging in broad discussions about the direction of health financing and organization. The Bank has staff with knowledge about trends in health insurance, for example, and relatively easy access to Ministries of Finance. The Bank’s health sector portfolio has for a long while included many projects designed to improve management information systems, logistics and supply chain functions, health worker training, policymaking at central and subnational levels, and other “systems” stuff. In contrast, the Global Fund and GAVI don’t focus on those areas, for the most part, and have limited policy-level contacts outside of the health sector. In its role as the <a href="https://web.archive.org/web/20090925035949/http://www.vaccineamc.org/">AMC (Advance Market Commitment)</a> Secretariat, the <a href="https://web.archive.org/web/20090925035949/http://www.gavialliance.org/">GAVI Alliance</a> Secretariat is seeking an additional member for its Independent Assessment Committee (IAC), this time with specific expertise in vaccine industry economics. With the formal launch of the <a href="https://web.archive.org/web/20090925035949/http://www.vaccineamc.org/updatejun_09.html">AMC for pneumococcal vaccines</a>, the IAC will oversee several core components of the AMC process; specifically, it reviews and approves the technical product profile (TPP) for the vaccine, determines AMC eligibility of vaccine candidates, and monitors information regarding the influence of the AMC on the development and production of specific vaccines. In addition, the IAC will establish when and if an adjustment of the pre?set long term price of vaccines is necessary and will have a dispute resolution function.</p> <p>The GAVI Alliance Secretariat is accepting applications from candidates with expertise in: clinical performance and delivery systems, contract law, public health, and public/private finance. Six donors (Italy, the UK, Canada, Russia, Norway and the Bill & Melinda Gates Foundation) have worked closely with the GAVI Alliance and the World Bank — along with many others who devoted countless hours to the design and implementation questions — to make the policy proposal a reality. To their great credit, they’ve posted draft reports from their two Working Groups so interested observers can see the where they’re going. Working Group 1 seeks to identify the health systems-related constraints to achieving global health goals, and presents estimates of costs of achieving priority goals (e.g. targeted reductions in maternal and child health). Working Group 2 (WG2) aims to identify new sources of funding and lay out the best options for channeling the funding to countries to improve health system performance.
